Is it safe to take creatine while on TRT?

Short answer

For most healthy men, yes. Creatine is not among the drug interactions on testosterone labels, and the International Society of Sports Nutrition considers it safe and well tolerated in healthy people at studied doses. The catch is lab work, because creatine supplements can raise blood creatinine and make your eGFR kidney estimate look worse than it is. Tell your provider so follow-up labs are read correctly.

Does creatine interact with testosterone?

No interaction is listed. The drug interactions section of the Depo-Testosterone label names oral blood thinners, oxyphenbutazone and insulin. Creatine is not on that list, and the two work through different paths: testosterone acts as a hormone, while creatine tops up the energy stores muscles use for short, hard efforts.

On safety, the International Society of Sports Nutrition position stand concludes that short- and long-term creatine use is safe and well tolerated in healthy people. That conclusion covers healthy users, which is why existing kidney problems change the answer.

Why does creatine change your kidney labs?

Because the main kidney test is built on creatinine, a waste product your muscles make. The National Kidney Foundation explains that your eGFR, the estimate of how well your kidneys filter, is calculated from blood creatinine along with age and sex.

The same source lists factors that raise creatinine and make eGFR appear lower than it really is: taking creatine supplements, eating large amounts of cooked meat, recent high-intensity exercise and high muscle mass. A lifter on TRT can tick every box at once. Testosterone may add lean mass, you may train hard the day before a draw, and creatine nudges the number further.

Factor Effect on creatinine What to do
Creatine supplement Raises it Tell your provider you take it
Hard workout in the last day or two Can raise it Schedule draws after a rest day when you can
Large cooked-meat meal Can raise it Follow any pre-test diet instructions
High muscle mass Raises it Ask whether cystatin C would help

How do you keep it from being misread?

Tell your provider before the draw, not after the result. MedlinePlus notes that you may be told not to eat meat for 24 hours before a creatinine test and that supplements can affect results, so list everything you take. Keeping your routine similar before each draw also makes changes over time easier to interpret.

When a creatinine-based eGFR still looks off, a second marker can help. The National Kidney Foundation says a cystatin C test may give a more accurate result in people with a lot of muscle, where creatinine levels can mislead. Our pages on the eGFR and creatinine kidney test and the cystatin C test explain both markers.

Who should be more careful?

Men with known kidney disease, diabetes or poorly controlled blood pressure should ask before starting creatine, since the safety evidence comes from healthy people. The same goes for anyone whose eGFR was already borderline before any supplement. Our page on supplements to stop on TRT covers other common products.

Is creatine worth taking alongside TRT?

That depends on your goals. If you lift and want an edge in short, intense efforts, creatine has a large body of research behind it. If your goal is energy or libido, it is not designed for that. Our guide to strength training for men over 40 on TRT puts supplements in context with training and recovery.

How Ultimate Male reads labs for men who train

Your provider looks at kidney numbers in context: your training, your supplements, your muscle mass and how the result has moved over time. A single high creatinine in a man who lifts and takes creatine is a reason to look closer, not a diagnosis. Draws are done on site in San Gabriel or Downey with results in 24 to 48 hours, and kidney markers are part of the clinic’s comprehensive and targeted blood panels. Can creatine replace TRT for low energy?
No. Creatine supports short, high-intensity efforts and is not a hormone treatment. If low testosterone is behind your symptoms, a supplement will not correct it.
Do I need to stop creatine before my kidney labs?
Not automatically. Some clinicians simply read the result with creatine in mind, while others ask for a pause before a recheck or order cystatin C instead. Your provider decides which approach fits.
Is creatine safe if I have kidney disease?
That needs an individual answer. The safety data cover healthy people, so men with reduced kidney function, diabetes or high blood pressure should ask before starting.
