How do I dispose of TRT needles and vials in California?

Short answer

Never put used needles in the trash, recycling or green-waste bin in California. Drop each one into a sharps container right away, close it when it is about three-quarters full, then use a free mail-back package from the state stewardship program or a household hazardous waste site. Leftover or expired testosterone, a controlled substance, goes to an authorized drug take-back location, not the sharps container.

Why can’t needles go in the trash in California?

Because loose sharps injure the people who handle your waste after it leaves the curb. In California, home-generated sharps are kept out of every household bin. LA County Public Works tells residents not to place sharps in the trash, green waste or recycling bin, and not to flush sharps or medicines down the toilet or drain. The good news is that the state requires a free option. CalRecycle explains that California’s pharmaceutical and sharps stewardship programs must provide safe, convenient disposal for home-generated sharps and medicines at no cost to the consumer, through a nonprofit organization created by manufacturers.

What counts as an approved sharps container?

A container built for sharps is the first choice. The FDA recommends a two-step process: put used needles and syringes into a sharps disposal container immediately after use, then follow your community’s rules for getting rid of the full container.

If you do not have one yet, LA County accepts a stand-in that will not leak, break or be punctured, as long as it is taped, sealed and labeled “SHARPS.” The county specifically rules out milk jugs, bleach bottles and soda bottles. Whichever you use, stop filling it at about three-quarters and keep it where children and pets cannot reach it.

Mail-back tends to suit men on weekly or twice-weekly injections, because the box arrives at home and leaves the same way. Call ahead before bringing a container to any site, since hours and accepted items change.

What about leftover or expired testosterone?

That goes to drug take-back, not the sharps container. The Depo-Testosterone label identifies testosterone as a Schedule III controlled substance. The FDA explains that authorized drug take-back locations, registered with the DEA, collect unused or expired medicines, including those that contain controlled substances, through on-site kiosks, drop-off boxes or mail-back programs.

California’s medicine stewardship program offers free drop-off locations and mail-back supplies too; CalRecycle points residents to its website and phone line. A vial with medicine still in it belongs in take-back. For an empty vial, ask the take-back site or your pharmacy whether they want it. What if you travel with needles?

Can I recap a needle before it goes in the container?
Drop it straight in instead. Handling a used needle more than you need to is how needle-stick injuries happen, which is why the FDA's first step is putting sharps in a container right after use.
Do alcohol swabs and empty packaging go in the sharps container?
No. Only items that can puncture skin belong there. Swabs, wrappers and gauze can go in regular household trash.
Can I take a full container on a plane?
It is easier to dispose of sharps before you travel or carry a small travel container and use mail-back when you get home. Check airline and airport rules before packing used sharps.
