What happens when you stop TRT?

Short answer

Once you stop TRT, blood testosterone falls back toward your old baseline within weeks, and sometimes below it for a while, because treatment quieted your brain's signal to the testes. Symptoms you were treating tend to return as levels drop. Your own production and sperm often take months to restart, and a provider may use HCG and timed labs to support that recovery.

How fast does testosterone drop after you stop?

Within days to a few weeks for most forms, and the pace depends on what you were taking.

There is a catch. While you were on TRT, your brain turned down LH and FSH, the hormones that tell the testes to work. What does the timeline look like, phase by phase?

The exact weeks vary with the form, the dose and how long you were treated, so treat this as a map rather than a schedule.

Will your symptoms come back?

If your low testosterone had a lasting cause, expect them to. Low energy, reduced sex drive, weaker morning erections, flat mood and slower recovery from training tend to return as levels fall. The testosterone label notes that men who took higher-than-recommended doses can have withdrawal symptoms lasting weeks or months, including depressed mood, fatigue, irritability, insomnia and low libido, while dependence at approved doses has not been documented.

If the cause was reversible and you have fixed it, such as excess weight, untreated sleep apnea or an opioid pain medicine, your own level may settle in a normal range. Some men stop because treatment never helped, and the AUA guideline suggests discussing exactly that when levels normalize at three to six months but symptoms do not improve. If low mood becomes severe or you have thoughts of harming yourself, call 911 or go to the nearest emergency room.

How long does natural production take to restart?

Months, and the range is wide. The AUA notes that most healthy men with normal testosterone recover sperm production after stopping, but there are no high-quality reports on recovery in men who were testosterone deficient to begin with.

The clearest numbers come from an analysis of 1,549 healthy men in hormonal contraception studies. Sperm returned to a fertile range in a median of 3.4 months, in 67% of men within 6 months, 90% within 12 months and all within 24 months. Recovery was faster with older age, shorter treatment and shorter-acting preparations.

Can HCG help you restart?

It can support the testes while the brain’s signal recovers. HCG acts like LH, and the Pregnyl label lists selected cases of hypogonadotropic hypogonadism in males among its uses. The AUA allows HCG, selective estrogen receptor modulators or aromatase inhibitors for men with low testosterone who want to keep fertility, though the evidence for specific restart plans is limited.

Is it dangerous to stop TRT suddenly?
The testosterone label states that dependence has not been documented at approved doses for approved uses. The main problem with a sudden stop is that symptoms can return before your own production restarts, so planning the stop and the follow-up labs makes the gap easier.
Should I stop TRT if I want to have a baby?
Talk to your provider before changing anything. Testosterone suppresses sperm, and in a large analysis of healthy men sperm took a median of 3.4 months to recover after stopping, so fertility plans often include HCG and a semen analysis.
