Why isn't Viagra working for me?

Short answer

Most often it is how the pill was taken. The AUA guideline cites studies in which incorrect use, such as no sexual stimulation or a large meal, explained 56% to 81% of treatment failures. Check the meal, arousal, timing, number of tries and dose first. If it still fails, low testosterone, diabetes, blood vessel disease or nerve damage may be blunting it, and those call for testing and often a different treatment.

Start here, because this is where most failures come from. The AUA guideline reports that in studies of men who said PDE5 pills did not work, incorrect use explained 56% to 81% of the failures. After men were re-taught how to use the medicine, 23.6% to 58.5% went on to succeed.

How many tries make a fair trial?

More than one, on separate days, with the timing and meal issues fixed. Nerves, alcohol or fatigue on a single night can sink any pill. The label limit of one dose per day still applies, so a fair trial takes time rather than extra tablets.

If you have taken it correctly on several occasions, at a dose your provider has adjusted, and still have little or no response, the problem is probably not technique.

What medical problems stop Viagra from working?

Some conditions undercut the way sildenafil works. The drug amplifies a signal that starts with arousal, nerves and healthy blood flow, so anything that weakens that chain can blunt it.

  • Low testosterone. The AUA recommends a morning testosterone level in every man with ED, and notes that in men with low testosterone, combining testosterone therapy with a PDE5 pill is more likely to work than the pill alone.
  • Diabetes. High blood sugar damages the nerves and blood vessels erections depend on, and NIDDK reports that more than half of men with diabetes develop ED.
  • Blood vessel disease. The AUA states that switching pills or changing dosing frequency will not help men with severe arterial insufficiency.
  • Nerve damage after prostate surgery. The American Cancer Society notes that these pills will not work if both nerves that control erections were damaged or removed.
  • Medicines and mood. Antidepressants, some blood pressure drugs, anxiety and depression can all interfere.

What comes next if it truly fails?

Several paths remain, chosen by the cause:

  • A different PDE5 medicine or schedule. The AUA allows dosing frequency changes or a different pill; see daily versus as-needed tadalafil.

  • Treating the cause. Confirmed low testosterone, uncontrolled diabetes or a medicine side effect can be addressed alongside ED treatment.
  • Investigational options. Shockwave therapy is not FDA-cleared for ED and is considered investigational by the AUA and SMSNA; PRP is not FDA-approved for ED, with mixed evidence.

Can Viagra stop working after it used to work?
Yes. The AUA notes that a significant share of men who respond at first become less responsive as ED progresses, which is a reason to look for an underlying cause rather than simply take more.
Should I take two pills if one does not work?
No. The label's maximum is 100 mg once a day. Taking extra raises side effects without much added benefit; a dose change is a decision for your provider.
Is generic sildenafil weaker than Viagra?
An FDA-approved generic contains the same active drug at the same strength. If a generic seems weaker, timing and meals are more likely culprits than the brand.
