High prolactin in men

Short answer

High prolactin suppresses the brain's signal to the testes, so testosterone and libido fall. Common causes in men are certain medicines, an underactive thyroid, kidney or liver disease, and a benign pituitary tumor called a prolactinoma. A high result should be repeated under the right conditions, and if it stays high without a clear cause, the next step is pituitary imaging and specialist care rather than starting testosterone.

How high prolactin shows up in men

Prolactin is a pituitary hormone known mainly for its role in milk production, but men make it too, and too much of it causes trouble. MedlinePlus lists these symptoms of high prolactin in men:

  • Low sex drive
  • Erectile dysfunction
  • Breast enlargement (gynecomastia)
  • Nipple discharge
  • Decreased facial and body hair

If a pituitary tumor is the cause, headaches and vision changes can join the list, as MedlinePlus notes for prolactinoma. The same page adds an important caution: there may be no symptoms at all, especially in older men. That is how high prolactin hides. A man is told he has low testosterone, and nobody asks why.

The AAFP review notes that high prolactin is less common in men than in women, and that larger tumors, called macroadenomas, are more common in men. A gradual loss of libido is easy to ignore or put down to age, which gives a slow-growing tumor time. Checking prolactin whenever testosterone comes back low, rather than waiting for headaches or vision changes, is how it gets caught earlier and while it is smaller.

How prolactin pushes testosterone down

Testosterone production starts in the brain. The hypothalamus signals the pituitary, the pituitary releases LH, and LH tells the testes to make testosterone. High prolactin interferes near the top of that chain, damping the signal so the pituitary releases less LH. The testes are healthy but underinstructed, and testosterone falls.

On lab work, that produces a recognizable pattern: low testosterone with LH that is low or inappropriately normal, the profile called secondary hypogonadism. MedlinePlus calls a prolactinoma the most common cause of high prolactin. The medicine list, though, is longer than most men expect. An AAFP review of galactorrhea names antipsychotics, including risperidone, and antidepressants as the drug classes most often involved, along with opioids, the antinausea drugs metoclopramide and domperidone, and others such as verapamil and methyldopa. The Endocrine Society’s hyperprolactinemia guideline devotes a section to assessing the cause and managing drug-induced cases.

Why a high result means a retest first

Prolactin moves during the day and responds to everyday things. MedlinePlus says the sample is usually taken three to four hours after you wake up, and you may be asked to fast. The AAFP review advises avoiding exercise and nipple stimulation for at least 30 minutes before the draw. A mildly high result after a hard workout, poor sleep or a stressful morning often looks different on repeat.

The AAFP review cites a male upper limit of about 18 ng/mL, while noting that values vary by laboratory, so your own report’s range is the one that counts. If a medicine might be responsible, any pause or switch before remeasuring is arranged by the clinician who recommended that medicine. Never stop a psychiatric or other medicine on your own. Why starting TRT is the wrong first move

When prolactin is truly high, testosterone therapy treats the downstream number while leaving the cause untouched. That matters for two reasons. First, if the cause is a pituitary tumor, it needs to be found and treated, and testosterone therapy could hide the symptoms that would have led to it. Second, when prolactin is brought down, the pituitary’s signal can recover and testosterone may come back on its own, without lifelong replacement.

So the path after a confirmed high prolactin with no clear medicine or thyroid cause is pituitary imaging. The AAFP review recommends pituitary imaging when the cause is unclear after lab evaluation and medication review, with contrast-enhanced MRI preferred. Treatment is then led by an endocrinologist. MedlinePlus notes that medicine is usually successful for prolactinoma, and the AAFP review identifies cabergoline as the preferred medicine. Symptoms that need emergency care

Get emergency care, by calling 911 or going to the ER, for a sudden, severe headache, especially with sudden vision loss, double vision, confusion, vomiting or drowsiness. Rarely, a pituitary tumor can bleed or swell suddenly, and that needs immediate treatment. Gradual changes in peripheral vision, such as bumping into door frames, deserve a prompt eye or medical exam.

Can stress or a workout raise my prolactin?
Slightly, yes. MedlinePlus notes that stress, exercise and even sex can cause small increases, and an AAFP review advises avoiding exercise and nipple stimulation for at least 30 minutes before the test. Large elevations need another explanation.
Is a prolactinoma cancer?
No. A prolactinoma is a noncancerous pituitary tumor. Larger ones can press on nearby structures, including the nerves to the eyes, which is why imaging and specialist follow-up matter.
Will my testosterone come back once prolactin is treated?
Often it can, because lowering prolactin releases the brake on the pituitary. How much recovers depends on the cause and on whether the pituitary itself was damaged, which is why testosterone is rechecked after prolactin is controlled.
