Tesamorelin is different: it is an FDA-approved drug made by Theratechnologies and sold as Egrifta WR, with a current label revised in March 2025.

It also sits in a different regulatory class. Tesamorelin is a protein-sized peptide, and on March 23, 2020 the FDA moved Egrifta onto its list of products deemed to be biologics. Biologics carry their own manufacturing and approval requirements, and with one company holding the approval, the price is set by that company rather than by competing makers. What does a month at the label dose involve?

At the label dose, a month of tesamorelin means a daily injection and four to five vials. The Egrifta WR label sets the dose at 1.28 mg under the skin once daily. Each vial is mixed once a week, provides daily doses for seven days, and must be discarded seven days after mixing, so a 30-day month runs through four full vials and part of a fifth.

That weekly mixing rule matters for budgeting. A skipped week does not stretch a vial, because the leftover solution has to be thrown away once the seven days are up. Your provider sets the dose and schedule; the label figures are context for reading a quote, not instructions.

Why insurance does not pay for body-composition use

Insurance coverage for tesamorelin is tied to its approved use, and body composition in men without HIV is not that use. The label approves Egrifta WR to reduce excess abdominal fat in adults with HIV who have lipodystrophy, and it states the drug is not indicated for weight loss management.

Health plans that list tesamorelin usually require prior authorization, meaning approval before the first fill, and that approval is checked against the labeled diagnosis. A request to shrink visceral fat in an otherwise healthy man falls outside it and is typically declined. Off-label use is legal when a licensed clinician judges it appropriate, but the cost then falls on you. What lab work comes with tesamorelin?

Tesamorelin needs more lab monitoring than most peptides, and those draws belong in your budget. The label warns that it raises IGF-1, asks clinicians to monitor IGF-1 during treatment, and says glucose intolerance or diabetes may develop, so glucose should be checked before and during therapy. In the original trials, 5% of patients on tesamorelin reached an HbA1c of 6.5% or higher by week 26, compared with 1% on placebo.
