Why do liver enzymes rise after a hard workout?
They rise because AST and ALT are not liver-only enzymes. MedlinePlus describes both as enzymes made mainly in the liver, and the word “mainly” matters: skeletal muscle holds them too. When a heavy session damages muscle fibers, the enzymes leak into your blood and a standard liver panel cannot tell where they came from.
The clearest evidence comes from a study of 15 healthy men who were active but not used to weightlifting. After a single one-hour lifting program, their AST, ALT, lactate dehydrogenase, CK and myoglobin all rose significantly and stayed raised for at least seven days. Every man analyzed had an AST above the upper reference limit on days three, four and five.
How long does the rise last, and what does the pattern look like?
The rise lasts at least a week, and ALT can lag behind AST. In the same study, ALT has a longer half-life than AST (about 47 hours against 17), so by days 10 to 12 most men had ALT above range while AST had settled. A draw taken then could look like a liver problem when it is really leftover muscle damage.

That last row is the useful clue. When AST and ALT are up but GGT, alkaline phosphatase and bilirubin look normal, the picture leans toward muscle rather than a liver that is struggling.
How does a CK test separate muscle from liver?
CK separates them because most of the body’s CK sits in skeletal muscle, not the liver. MedlinePlus explains that a small amount reaches the blood from everyday wear and tear, while damaged muscle releases much more. A high CK next to a high AST, especially with AST higher than ALT, is the classic training footprint.
Two details shape how CK is read. Normal CK varies with age, sex, race, muscle mass and how active you are, so a lifter’s baseline runs higher than a desk worker’s. CK may also not peak until up to two days after an injury, so one draw can catch it on the way up. If CK is normal but ALT is still high after rest, the liver deserves a closer look. What is a rest-week redraw?
A rest-week redraw is a repeat test after about seven days without heavy lifting. The study authors suggested restricting weightlifting for at least one week before blood tests in clinical research, and the same logic works for you. MedlinePlus adds that providers may ask you to skip intense exercise and alcohol for a few days before a CK test.
On the redraw, the useful markers are AST, ALT, CK and GGT together. The American College of Gastroenterology guideline puts a truly healthy ALT at about 29 to 33 IU/L for men, lower than the upper limit many labs print, so ranges differ from one report to the next. When is a high result more than training?
A high result is more than training when ALT stays up after rest, when GGT or bilirubin are also abnormal, or when you take something that strains the liver. Some symptoms need urgent care instead of a redraw. Severe muscle pain, swelling, weakness or dark, cola-colored urine after an extreme workout can signal rhabdomyolysis, which MedlinePlus notes can damage the heart and cause sudden kidney failure.
