What can raise PSA temporarily

Short answer

PSA can rise for a short time after ejaculation, vigorous cycling, a prostate or urinary tract infection, a prostate biopsy, cystoscopy or a urinary catheter. The National Cancer Institute advises avoiding activities that raise PSA for 2 days before testing, and notes that infection or a biopsy can keep PSA raised for a month or two. A borderline result is usually repeated before any next step.

What can push PSA up for a short time?

PSA is a protein made by the prostate, and anything that irritates, presses on or inflames the gland can release more of it into the blood for a while. That means a single high reading can reflect what happened in the days before the draw rather than a lasting change.

The common short-term triggers, and how long the major sources suggest waiting, look like this:

How do ejaculation and cycling raise PSA?

Both put mechanical pressure on the prostate. MedlinePlus asks men to avoid sex and masturbation for 24 hours before a PSA test, because releasing semen can raise the level and make the result less accurate.

Cycling is less certain but worth respecting. The American Cancer Society notes that some studies suggest cycling may raise PSA for a short time, possibly from saddle pressure, though not all studies agree. The National Cancer Institute names vigorous exercise such as cycling alongside ejaculation and recommends avoiding both for 2 days before testing. If you ride long distances on weekends, book your draw for midweek.

Why do infections and prostatitis matter so much?

Inflammation has the largest and longest effect of the everyday triggers. An infection or inflammation of the prostate can keep PSA raised for a month or two, according to the National Cancer Institute, and MedlinePlus lists urinary tract infections among the noncancer causes of a high PSA.

That has a practical consequence. If you have burning, urgency, pelvic pain or a fever around the time of your test, mention it before reading anything into the number. The MedlinePlus encyclopedia notes that men with a high PSA are often retested, most often within 3 months, sometimes after treatment for a prostate infection first.

Do prostate exams and procedures affect the result?

Procedures that physically disturb the prostate or bladder can. MedlinePlus lists recent cystoscopy, prostate biopsy, a recently placed bladder catheter and a recent colonoscopy among things that can raise PSA, and says not to test soon after urinary procedures.

The digital rectal exam is the mildest of these. The American Cancer Society says some studies found a slight rise after a rectal exam and others found none. Because of that uncertainty, some doctors draw the PSA before doing the exam when both happen at the same visit, which is a simple way to take the question off the table. Can testosterone therapy raise PSA?

Yes, and this is not a short-term blip in the same sense. The American Cancer Society notes that taking testosterone, or medicines that raise testosterone, may cause a rise in PSA. Men on treatment are monitored for that rise, and the question is how large and how fast it is. Our page on a rising PSA on TRT covers how that monitoring works.

How long should you wait before a recheck?

It depends on the trigger. For ejaculation and cycling, 48 hours of avoidance before the draw covers the guidance from all three sources. For an infection, prostatitis or a biopsy, the wait is longer because PSA can stay raised for a month or two, and the infection is usually treated first.

For an unexplained high reading with no obvious trigger, the National Cancer Institute says a doctor may recommend a repeat PSA in 6 to 8 weeks to confirm it. If the repeat comes back normal, the first number was probably noise. If it stays high or keeps climbing, a follow-up test such as free PSA or a urology referral is the next step. Can dehydration or a heavy meal raise PSA?
Neither appears on the standard lists of short-term PSA triggers from MedlinePlus, the National Cancer Institute or the American Cancer Society. Fasting is not usually required for PSA, though it may be for other tests drawn at the same time.
Do medicines ever lower PSA instead?
Yes. Finasteride and dutasteride, used for an enlarged prostate or hair loss, lower PSA, so a result on these medicines is read against a lower cutoff. Tell whoever reads your result if you take either one.
Should I stop having sex before every PSA test?
