Why is HCG given more often than testosterone?
HCG is given on a steady rhythm because its job is to keep the testes working between testosterone doses. Testosterone from outside the body tells the pituitary to stop sending its own signals, LH and FSH, and without them the testes slow down. The American Society for Reproductive Medicine explains that this is how testosterone use can lower sperm production.
The size of that shutdown is striking. In a study of healthy men given weekly testosterone enanthate, LH and FSH fell to 5% and 3% of baseline, and testosterone inside the testes fell by 94%. HCG acts like LH, so giving it regularly replaces the missing signal.
What schedules have been studied?
The schedules with the clearest evidence use HCG every other day alongside testosterone. The label schedules are different because they describe HCG used on its own.
| Source | HCG schedule | Context |
|---|---|---|
| Coviello and colleagues, 2005 | 125, 250 or 500 IU every other day | Healthy men on weekly testosterone enanthate for 3 weeks; 250 IU kept testicular testosterone within 7% of baseline |
| Hsieh and colleagues, 2013 | 500 IU every other day, into the muscle | 26 men on weekly injections or daily gel; none became azoospermic, and 9 contributed to a pregnancy |
| Pregnyl label, one regimen | 500 to 1,000 units 3 times a week for 3 weeks, then twice a week for 3 weeks | HCG alone for hypogonadotropic hypogonadism |
| Pregnyl label, other regimen | 4,000 units 3 times a week for 6 to 9 months, then 2,000 units 3 times a week | HCG alone for hypogonadotropic hypogonadism |
These are context, not instructions. The dose you use, and whether it is two, three or more times a week, is set by your provider from your goals and labs.
How do I fit HCG and testosterone into one week?
You fit them together by picking fixed days for HCG first, then placing the testosterone shot. Nothing in the studies above shows that giving HCG on the same day as testosterone, or on a different day, changes results; what they kept constant was a regular rhythm.
Examples of how a provider might lay out a week:
| Testosterone schedule | HCG three times a week | HCG twice a week |
|---|---|---|
| Once weekly, Monday | Monday, Wednesday, Friday | Monday and Thursday |
| Twice weekly, Monday and Thursday | Monday, Wednesday, Friday | Monday and Thursday |
Pairing HCG with a testosterone day means fewer days to remember. If both fall on the same day, use separate syringes unless your provider has told you otherwise; our page on mixing HCG and testosterone in one syringe explains why. A phone reminder or a paper calendar on the refrigerator door does more for consistency than any particular weekday.
How should HCG be stored between doses?
HCG needs more care than testosterone once it is mixed. According to the Pregnyl label, the unopened vial is kept at room temperature, but the mixed solution goes in the refrigerator, must not be frozen or shaken, and is discarded after 60 days. The label lists the injection as intramuscular; your provider tells you which route and needle to use. Our guide on how to mix and inject HCG walks through each step.
How Ultimate Male sets up HCG with TRT
Ultimate Male uses Pregnyl, the brand-name HCG, because compounded HCG is no longer available since HCG became regulated as a biologic in 2020. Your schedule is built around your goals, whether that is preserving fertility, keeping testicular size or both, and written out so you know exactly which days to inject.
Follow-up labs track testosterone and estradiol so the HCG dose can be adjusted; our page on lab monitoring on HCG explains what is checked. To learn more, read about HCG with testosterone therapy or start with the free call about testosterone therapy.
