How often can I use Trimix?

Short answer

The common limit is no more than three injections a week, with at least 24 hours between doses. Compounded Trimix has no FDA label of its own, so that rule comes from the labels for alprostadil, its FDA-approved ingredient. Spacing doses, rotating sides and regular checkups are the safeguards against penile scarring, which can cause lumps or a bend. Your provider sets your exact limit.

Where does the three-times-a-week rule come from?

From the FDA-approved versions of the main ingredient. Trimix combines alprostadil, papaverine and phentolamine, and only alprostadil is sold as an approved injection on its own. MedlinePlus says alprostadil injection should not be used more than three times a week, with at least 24 hours between uses. The CAVERJECT and EDEX labels give the same limit.

Providers apply it to compounded blends because there is no separate label to follow. Memorial Sloan Kettering’s penile injection therapy guide gives its own patients the same rule: up to three times a week, 24 hours apart, and never more than one injection in 24 hours.

Why do spacing and rotation matter?

Because repeated injections can leave scar tissue, called fibrosis, inside the penis. The CAVERJECT label reports penile fibrosis in 3% of men across its clinical studies, and 7.8% in one self-injection study lasting up to 18 months. The EDEX label reports 7.8% for fibrosis, including Peyronie’s disease, in studies of up to 24 months. Scarring can show up as a firm lump or a curve in the erect penis.

Be clear about what the evidence does and does not show. The AUA erectile dysfunction guideline notes that fibrosis and deformity have been reported with all injection medicines, with no single drug or combination clearly worse, and that there are no reliable predictors of who will develop it. That is why the safeguards work as a package: a frequency ceiling, rotating sites, documenting any existing plaque before you start, and regular exams afterward. What signs should you report?

Anything new in the shape or feel of the penis. The CAVERJECT label asks men to report new or worsening penile pain, nodules or hard tissue, and curvature of the erect penis as soon as possible. MedlinePlus adds redness, swelling or tenderness and unusual curving among the serious side effects of alprostadil that need a prompt call.

The labels advise stopping injections if a bend or fibrosis develops, and EDEX notes that treatment can resume if the change settles. That decision belongs to your provider after an exam.

What counts as one use?

One injection, with its own 24-hour clock. If the erection was too weak, the answer is a call to your provider about the dose, not a top-up. Keeping a simple log of the date, the dose and the quality and length of each erection helps your provider fine-tune things at checkups. Can you use an ED pill on the days in between?

Only if your provider says so, and with spacing they set. Combining injections and pills raises the risk of a prolonged erection. MSK, for example, tells its patients not to take sildenafil within 18 hours of an injection and not to inject within three days of a 10 or 20 mg tadalafil dose. Can I inject twice in one night if the first dose wears off?
No. One injection per 24 hours is the rule, and a second dose the same day raises the risk of a prolonged erection. If a dose regularly runs short, your provider adjusts it.
Is three times a week a target?
No, it is a ceiling. Using injections less often is fine, and nothing is gained by reaching the maximum.
Do I need regular checkups if everything is fine?
Yes. The CAVERJECT label asks men on home injections to see their provider about every three months so the penis can be examined for early changes.
