What does the label say about a second dose?
One dose in a day is the ceiling. The Viagra label states that the maximum recommended dosing frequency is once per day. The usual dose is 50 mg taken about an hour before sex, though it can be taken anywhere from 30 minutes to 4 hours ahead.
Depending on how well it works and how you tolerate it, the dose may be raised to 100 mg or lowered to 25 mg. MedlinePlus puts the same rule in plain terms: sildenafil usually should not be taken more than once every 24 hours. Your provider sets your dose within those limits.
Why is 100 mg the top dose?
Higher doses add side effects faster than they add benefit. In the label’s fixed-dose trials, abnormal vision, mostly a color tinge, was reported by 1% of men at 25 mg, 2% at 50 mg and 11% at 100 mg.
The gain in erections is smaller than doubling the dose suggests. Across studies reviewed in the American Urological Association guideline, men on 50 mg improved their erectile function scores from an average of 14.0 to 22.6, and men on 100 mg went from 14.4 to 23.8. A second 100 mg pill would push you beyond anything studied.
Sildenafil also lingers longer in some men. The label reports that blood levels 24 hours after a dose run 3 to 8 times higher than usual in men over 65, men with liver disease or severe kidney disease, and men taking certain antibiotics or antifungals. That is why a starting dose of 25 mg is considered for them.
What should you do when one dose is not enough?
Check how the dose was taken before changing anything. A weak response often traces back to something fixable:

Is there an option for a longer window than one pill gives?
Yes, and it is a different medicine rather than a second sildenafil. The Cialis label reports that tadalafil improved erections for up to 36 hours after a dose, and it also comes as a low daily dose of 2.5 or 5 mg taken regardless of when sex happens.
