How should I store Trimix?

Short answer

Keep Trimix cold and follow the label on your vial, because the compounding pharmacy sets the storage temperature and the beyond-use date. The alprostadil in the mix breaks down even when chilled; in one study it fell to about 70% of its starting strength after 60 days in the refrigerator. Travel with it in an insulated bag with ice packs and declare it at airport security.

Why does Trimix have to stay cold?

Trimix stays cold because one of its three ingredients is fragile. Papaverine and phentolamine hold up well in solution, but alprostadil (prostaglandin E1) slowly breaks down, and the other two speed that up.

A stability study of these mixtures stored them at refrigerator temperature (2 to 8°C) for up to 60 days. Papaverine and phentolamine stayed within a few percent of their starting strength. Alprostadil did not: in the three-drug mix it fell to about 70% of its starting concentration by day 60, with the steepest drop in the first 10 days. Papaverine had the greatest effect on that breakdown.

That is why a vial that worked well in week one can feel weaker later, even when it has been stored correctly.

The approved single-drug injections behave differently. The Edex label describes a freeze-dried powder in a two-chamber cartridge, mixed with saline right before use and stored at room temperature. The Caverject label allows room-temperature storage for its 20 mcg vials. Trimix arrives already in solution, so the clock starts when the pharmacy makes it.

Refrigerator or freezer: which is right for your vial?

The answer is on your pharmacy label, and it can differ from one pharmacy to the next. Compounded Trimix is made for you individually, and the pharmacy assigns a beyond-use date based on how it was prepared and where it will be kept. Some mixes are labeled for the refrigerator, others for the freezer with one vial thawed at a time.

Two questions are worth asking directly: can a thawed vial go back into the freezer, and does the beyond-use date change once you start using it? Do not assume the answer for one pharmacy applies to another.

How quickly does Trimix lose strength when it warms up?

There is no reliable published figure for how fast compounded Trimix loses potency at room temperature, and any exact number you see online is a guess. What the evidence does show is that alprostadil degrades even in the refrigerator, and warmth generally speeds up chemical breakdown. Treat a vial that sat out for hours, or rode in a hot car, as suspect.

Signs to watch for:

  • A firm response from your usual dose turns noticeably weaker
  • The solution looks cloudy, discolored or has particles
  • The vial is past its beyond-use date

How do you travel with Trimix?

Keep it cold and keep it with you. An insulated bag with frozen gel packs works for most trips, and the vial should ride in your carry-on rather than a checked bag, where temperatures are unpredictable.

At the airport, TSA rules are on your side. The agency’s medical conditions page says larger amounts of medically necessary liquids are allowed if you declare them, and that ice packs and gel packs used to keep medical items cool may be frozen, partially frozen or melted at screening. TSA also allows unused syringes when they travel with injectable medication, and recommends keeping the pharmacy label on.

A few more habits help:

  • Ask the hotel for a room refrigerator before you arrive
  • Pack a sharps container or plan how you will dispose of used syringes
  • Know where the nearest emergency room is at your destination, in case of an erection lasting four hours
