The quick verdict
Sildenafil suits men who can plan: date night, a weekend away, a predictable routine. It has been used for decades, comes as an inexpensive generic, and leaves your system sooner, which some men prefer. Tadalafil suits men who want to stop watching the clock, either with an as-needed dose that covers a day and a half or a low daily dose that keeps things ready all the time.
Neither is stronger in a meaningful sense. The difference is timing, food and side effects.
A few situations tip the balance quickly. If you take an alpha-blocker for your prostate, your provider will choose and time either medicine carefully because of blood pressure. If you have both erectile dysfunction and urinary symptoms from an enlarged prostate, daily tadalafil is FDA-approved for both. If you have tried one at a single dose, once, after a big dinner and a few drinks, you have not really tried it yet.

How does each work?
Both are PDE5 inhibitors. During arousal, your body releases a signal that relaxes the smooth muscle in penile blood vessels so blood can flow in; PDE5 is the enzyme that switches that signal off. Blocking it keeps the signal going longer. Neither creates an erection on its own, which is why arousal is still required.
The real differences come from how long each one stays active in the body. The Viagra label gives a usual timing of about 1 hour before sex, with a window from 30 minutes to 4 hours. The Cialis label reports improved erectile function compared with placebo up to 36 hours after a dose, with a mean half-life of 17.5 hours.
Why a heavy meal matters for sildenafil
Food changes sildenafil’s timing; it does not change tadalafil’s. On the Viagra label, taking sildenafil with a high-fat meal delayed its peak level by a mean of 60 minutes and lowered that peak by 29%. A steak dinner followed by a tablet can mean a slow or weaker effect that men mistake for the medicine failing.

Side effects: tinted vision vs back pain
Both commonly cause headache, flushing, nasal congestion and indigestion. Each also has a signature effect:
- Sildenafil and vision. In flexible-dose trials on the label, abnormal vision occurred in 3% of men, mostly a mild, temporary color tinge, light sensitivity or blur. It is linked to sildenafil’s weaker selectivity for an enzyme in the retina. See why Viagra can tint vision blue.
- Tadalafil and back pain. In as-needed trials, back pain occurred in up to 6% of men on the 20 mg dose. The label notes it usually starts 12 to 24 hours after a dose and resolves within 48 hours. See Cialis and back pain.
Both can drop blood pressure, especially with alcohol or alpha-blockers, and both are dangerous with nitrates. MedlinePlus tells you to inform your provider about any nitrate for chest pain, and the tadalafil entry adds riociguat. Planned or spontaneous sex: convenience and cost factors
Your routine is the deciding factor. If sex happens on predictable nights, sildenafil is simple and short-acting. If it happens whenever the mood strikes, or if you also have urinary symptoms from an enlarged prostate, daily or as-needed tadalafil removes the timing problem.
