Is TRT legal in California?

Short answer

Yes. Testosterone therapy is legal in California when a licensed clinician evaluates you, documents a medical reason and authorizes the medicine. Testosterone is a federal Schedule III controlled substance, every dispensed fill is reported to a state tracking database, and ordering it online without an appropriate exam is illegal. Remote-only starts also depend on federal telemedicine rules that run through December 31, 2026.

Three things: a licensed clinician, an appropriate evaluation and a documented medical reason. Testosterone is a federal Schedule III controlled substance, a status the Depo-Testosterone label states in its section on abuse and dependence. That puts it in the same legal family as other controlled medicines, so the path from evaluation to pharmacy is tightly defined.

What falls outside that path is not legal medicine in California. Testosterone bought from a website that never examined you, or from a gym contact, skips every safeguard, and our answer on self-sourced testosterone explains the health risks that come with it.

Why must an evaluation and diagnosis come before treatment?

California law requires it. The Medical Board of California calls the appropriate-examination requirement in Business and Professions Code section 2242 the cornerstone of why ordering medicines online without a real clinical relationship is illegal. A separate section, 2242.1, bars furnishing dangerous drugs over the internet without an appropriate prior exam and a medical indication.

The medicine’s own label sets a clinical bar too. Before starting Depo-Testosterone, the label tells clinicians to confirm the diagnosis with morning testosterone measured on at least two separate days, both below the normal range. Can you start TRT through telehealth in California?

Sometimes, but the rules come from two levels of government.

The Federal Register notice for the fourth extension explains that once a clinician has seen you in person once, the Ryan Haight limits stop applying to that relationship, even if the visit was for something else. What does California add on top of federal law?

Tracking. California’s Controlled Substance Utilization Review and Evaluation System, run by the Department of Justice, records every Schedule II through V controlled substance dispensed in the state, testosterone included. Clinicians who can authorize those medicines must register for access, and Health and Safety Code section 11165.4 sets the rules for when they must consult the database.

The same page notes that any written order for a Schedule II through V medicine must be on a tamper-resistant form from a state-approved printer. For patients, the practical effect is simple: your testosterone history follows you between pharmacies, and early or duplicate fills are visible to the people managing your care.

Is it legal to buy testosterone online in California?
Only through a licensed clinician who has examined you and found a medical reason. The Medical Board of California notes that state law bars furnishing dangerous drugs over the internet without an appropriate prior exam and medical indication, and a violation can bring a citation or a civil penalty of $25,000 per occurrence.
Is there a limit on how many refills I can get?
Yes. Federal rules allow an order for a Schedule III medicine such as testosterone to be refilled no more than five times, and not beyond six months from the date it was issued. After that, a new authorization is needed.
Does the federal telemedicine deadline affect men already in care?
It affects relationships that began remotely without any in-person evaluation. Once the clinician managing your testosterone has seen you in person at least once, the federal in-person requirement is already met for that relationship.
