Why did I gain weight after starting TRT?

Short answer

In the first weeks, extra pounds on TRT are usually water, because testosterone makes the body hold on to more salt and water. Over the following months, lean muscle can add weight while fat drops, so the scale may barely move even as your shape changes. A body composition scan separates fat from lean mass, which tells you far more than the scale does.

Is early weight gain on TRT usually water?

Usually, yes. A quick jump on the scale in the first weeks is most often fluid, not fat. The Depo-Testosterone label lists retention of sodium, chloride and water among its adverse reactions, and the MedlinePlus testosterone injection page lists weight gain as a common side effect.

Water weight tends to show up fast and move around from day to day. Fat and muscle change more slowly. If the scale jumped within a few weeks of your first dose, water is the first suspect.

How can you tell water from muscle or fat?

The timing, the location and what is happening in the gym all give clues.

The research on timing fits this table. A review of when testosterone’s effects appear found that changes in fat mass, lean mass and strength occur within 12 to 16 weeks, and that body weight often changes little because fat loss and lean gain offset each other. The Endocrine Society guideline likewise describes testosterone increasing fat-free mass and reducing whole-body and abdominal fat.

What does a body composition scan show?

A body composition scan splits your weight into fat mass and lean mass, which is the question the scale cannot answer. Two men can gain the same 4 pounds with opposite stories: one added fluid and a little fat, the other added muscle and lost fat. The scan shows which story is yours.

It is most useful as a series. A baseline reading before you start TRT, then repeat readings every few months using the same method, show the direction of change far better than any single number. Could TRT be making me gain fat?

Trials point the other way: on average, testosterone shifts the body toward more lean mass and less fat. When the waist grows on TRT, the usual suspects are the ones that apply to anyone, such as eating more to fuel extra training, alcohol and less sleep.

It is still worth asking whether your dose is part of the picture. When is weight gain a warning sign?

Fluid that keeps building is not a cosmetic issue. The Depo-Testosterone label warns that swelling, with or without heart failure, can be a serious complication in men who already have heart, kidney or liver disease, and MedlinePlus lists swelling of the hands, feet, ankles or lower legs as a side effect to report right away.

Call the clinic the same day for swelling that is spreading or pitting, or a gain of several pounds in a few days. Shortness of breath, especially when lying flat, or chest pain needs 911 or the nearest emergency room.

Will the water weight go away on its own?
It may settle, but swelling that persists or grows should be reported. Your provider may check your blood pressure, review your dose and look at other medicines that cause fluid retention.
