What happens if I miss a semaglutide dose?

Short answer

For weekly Wegovy, the label says to inject the missed dose as soon as you can if your next scheduled dose is more than 48 hours away, and to skip it if the next one is closer. If you miss two or more weekly doses in a row, the label calls for restarting the dose climb at a lower step to avoid a rebound of nausea and vomiting.

What does the label say about one missed weekly shot?

The answer turns on how close your next scheduled injection is. The Wegovy label gives a simple 48-hour rule for a single missed dose of the weekly injection:

  • If your next scheduled dose is more than 2 days (48 hours) away, inject the missed dose as soon as possible.
  • If your next scheduled dose is less than 2 days away, skip the missed dose.
  • Either way, go back to your usual injection day the following week.

Here is how that plays out for a man whose regular injection day is Sunday:

Do not double up to catch up. MedlinePlus says plainly not to take a double dose to make up for a missed one, because a bigger hit to the stomach is the likeliest result.

What if you have missed two or more doses in a row?

Two or more consecutive missed doses change the plan. The current Wegovy label says that if two or more weekly doses in a row are missed, the dose escalation should be restarted at a lower dose to reduce the risk of stomach side effects.

The reason is tolerance. Semaglutide has an elimination half-life of about one week, according to the label, so each week without a shot leaves roughly half as much medicine on board. The tolerance your stomach built to nausea, fullness and slowed digestion fades along with it. Jumping straight back to 1.7 mg or 2.4 mg after a two- or three-week gap can bring back the vomiting and diarrhea many men had during the first month.

How far back to step is your provider’s call. It depends on how long the gap was, which dose you were on and how rough your first escalation felt. Are the rules different for Ozempic, the tablet or compounded semaglutide?

Yes, and that catches people out. The window depends on the product and the reason you take it:

  • Ozempic for type 2 diabetes: MedlinePlus lists a wider window, giving the missed dose within 5 days and skipping it if more than 5 days have passed.
  • Wegovy tablets: the label says to skip a missed daily tablet and take the next one the following day. Our comparison of oral semaglutide and injections explains how the two forms differ.
  • Compounded semaglutide: there is no FDA-approved label, so the clinician managing it should tell you which rule to follow and what concentration is in your vial.

Can you move your injection day or time?

The time of day is flexible. The Wegovy label says both the time of day and the injection site can be changed without changing the dose, so a morning shot can become an evening shot. The label otherwise asks for the same day each week.

If your schedule has shifted for good, for example a new work rotation, ask your provider how to move to a new day without leaving too short a gap between two doses. When should a missed dose prompt a call?

Call or message your provider before your next shot if any of these apply:

  • You have missed two or more weekly doses in a row.
  • You missed because of vomiting, diarrhea or another illness.
  • You are planning surgery or a procedure with sedation; see stopping semaglutide before surgery.
  • You cannot tell which dose you should restart on.

Some symptoms are not a scheduling question. Severe belly pain that will not go away, with or without vomiting, can signal pancreatitis or a gallbladder attack: go to the emergency room. Will one missed dose undo my progress?
Unlikely. Semaglutide has a half-life of about a week, so some medicine is still working after a late or skipped shot. Get back on your regular day rather than trying to make up for it.
Do I have to go all the way back to 0.25 mg?
Not necessarily. The label says to restart the escalation at a lower dose, and your provider picks the step based on how long you were off and how you handled your last dose.
What if I skipped a dose because I was sick?
Tell your provider before the next shot. Vomiting or diarrhea can leave you dehydrated, and restarting while you are still unwell can make it worse.
