What does the STEP 1 curve show at 12 weeks?
It shows an average loss of about 6% of body weight by week 12. STEP 1 enrolled 1,961 adults with obesity, or overweight plus a related condition, and no diabetes. Why is the first 3 months only about 40% of the total?
Because you are not on the full dose yet. The Wegovy label starts at 0.25 mg and steps up every four weeks: 0.5 mg at week 5, 1 mg at week 9, 1.7 mg at week 13 and the 2.4 mg maintenance dose from week 17. Week 12 falls at the end of the 1 mg block. The early steps exist mainly so your stomach can adjust, and the label lets a step be delayed four weeks if a dose is not tolerated.
So six percent by week 12 against roughly 15 percent by week 68 means about 40 percent of the eventual loss happens in the first quarter of treatment, on less than half the maintenance dose. The curve keeps falling steeply through month seven and then flattens toward the end of the first year.
What changes the result for one person?
Averages hide a wide spread. In STEP 1, about 86% of semaglutide participants weighed at week 68 had lost at least 5%, and about a third lost 20% or more, while some lost very little. Factors that shape the first three months include:
- Whether you tolerate each step on time or need to pause one
- How much your eating actually changes, especially portion size and alcohol
- Starting weight, since the same percentage means more pounds for a heavier man
- Muscle and water shifts, which can make the scale jump around week to week
A 20-week run-in in the STEP 4 trial gives another anchor. People who reached the full 2.4 mg dose had lost an average of 10.6% by week 20, which fits the STEP 1 curve.
How should you judge progress at 3 months?
Judge the direction and the dose, not one number. A slower start on a held dose step is common and is not the same as a poor response. Your provider will usually look at weight, waist, how you feel at each dose and whether muscle is being protected.
