Why does HCG cost more than it used to?
HCG costs more because the lower-cost compounded version is gone. On March 23, 2020, chorionic gonadotropin products including Pregnyl and Novarel moved onto the FDA’s list of products deemed to be biologics, changing the legal category they sit in.
That change ended compounding. The FDA’s compounding questions and answers put it in one word: asked whether biologics can be compounded, the agency answers no, because federal law has no pathway for biologics prepared outside an approved license. Men who once bought lower-cost compounded HCG now use licensed products made by the drug’s manufacturer, priced as brand medicines. What does a month of HCG involve?
A month of HCG is set by one number, the units per week, and by how fast that empties a vial. The Pregnyl label describes a 10,000 USP unit multiple-dose vial, mixed with the supplied diluent, that stays stable for 60 days in the refrigerator after mixing.
For selected men with hypogonadotropic hypogonadism, the label lists regimens that range from 500 to 1,000 units three times a week up to 4,000 units three times a week. At 500 units three times a week, one vial covers about six and a half weeks, inside the 60-day window. At 4,000 units three times a week, a vial lasts less than a week. Use alongside testosterone therapy is off-label, and your provider sets the dose; these label figures only show why two HCG quotes can differ so widely.

Does insurance ever cover HCG?
Insurance sometimes covers HCG, but almost always for its labeled uses and only after approval. The Pregnyl label lists uses including prepubertal cryptorchidism and selected cases of hypogonadotropic hypogonadism in males, and plans that cover it generally require prior authorization tied to those diagnoses.
Using HCG alongside testosterone to keep the testes active is off-label, and plans that limit coverage to labeled diagnoses may decline it on that basis. Fertility coverage also varies widely by plan and employer.
