Why get blood work after a cycle?
Because how you feel after a cycle does not tell you what is happening inside. Anabolic steroids switch off your own testosterone production, and an Endocrine Society scientific statement notes that after long use this suppression can last for months, in some men for years, and that some never regain normal levels. The same statement describes dose-related rises in hemoglobin and hematocrit, and a fall in HDL cholesterol with a rise in LDL.
SARMs are not a gentler shortcut. The FDA warns that SARMs are not approved drugs and lists heart attack and stroke risk, liver injury, infertility and testicular shrinkage among the reported harms. Our article on SARMs and FDA warnings covers the regulatory side.
What belongs in an honest post-cycle panel?
A good post-cycle panel answers two questions: is your own production coming back, and did the cycle leave anything that needs attention?

The enzyme point matters for anyone who lifts hard. The Endocrine Society statement notes that muscle breakdown from heavy workouts can raise transaminases and be mistaken for liver damage. When should you draw it?
Timing depends on what you used. A panel drawn while compounds are still active mostly measures the drugs, not your recovery, so the hormone part of the panel is usually timed for after they have cleared. Long-acting injectable compounds take longer to clear than short-acting ones.
A sensible pattern is a first panel once the last compound should be gone, then a repeat of LH, FSH and testosterone a couple of months later to see the trend. Hematocrit, lipids and liver enzymes can be checked on the first draw regardless, because those reflect the cycle itself. Be specific with your provider about compounds, doses and dates; vague answers make results harder to read.
What are the signs natural production is not recovering?
The clearest lab sign is LH and testosterone that stay low months after you stopped. In a study of former long-term steroid users, five men still had testosterone below 200 ng/dL after 3 to 26 months without steroids, and untreated former users as a group had smaller testes and lower sexual desire than weightlifters who had never used them.
Symptoms that fit include:
- Low libido or erection problems that persist after the cycle
- Fatigue, low drive or low mood
- Testicles that stay noticeably smaller
- Strength falling away beyond what stopping the cycle explains
- Breast tenderness or swelling
Low mood deserves particular attention. In the same study, 7 of 24 former users (29%) had a major depressive episode during withdrawal. If you have thoughts of harming yourself, call or text 988 right away, or call 911. That comes before any lab visit.
What can treatment look like?
Treatment depends on whether you want to restore your own production, protect fertility, or simply feel well again. The AUA guideline notes that HCG, alone or with a selective estrogen receptor modulator, has been shown to help restart testosterone and sperm production in men who previously used anabolic steroids, and that return of sperm can take up to two years.
If production does not recover, testosterone therapy at normal, monitored levels is a medical option, with the same labs and safety checks as any other TRT plan.
