What enclomiphene does
Enclomiphene citrate turns up your own testosterone production instead of replacing it. It blocks estrogen’s feedback signal in the hypothalamus and pituitary, the brain centers that decide how hard the testes should work. With that brake eased, the pituitary releases more luteinizing hormone (LH) and follicle-stimulating hormone (FSH). LH drives testosterone production in the testes, and FSH supports sperm production.
That is the key difference from testosterone therapy. Testosterone from injections, capsules, cream or pellets tells the brain there is plenty on board, so LH and FSH fall and sperm production usually drops. In two phase 3 trials of overweight men aged 18 to 60 with low testosterone and low or normal LH, enclomiphene raised testosterone, LH and FSH while sperm concentrations stayed in the normal range, whereas men using testosterone gel saw sperm counts fall.
Enclomiphene has never been FDA-approved, so the clinic’s supply comes from a licensed compounding pharmacy. The FDA is plain that compounded drugs are not FDA-approved and that the agency does not verify their safety, effectiveness or quality before they are marketed. Our page on enclomiphene’s history and 2026 status covers the approval story in detail.
Who it suits, and who should not take it
Enclomiphene tends to suit men who:
- have confirmed low testosterone with LH that is low or in the normal range, the pattern called secondary hypogonadism;
- want children now or later and need to keep sperm production going;
- would rather take a pill than use needles, cream or pellets;
- carry extra weight, the group the main trials studied.
The AUA guideline states that testosterone should not be given to men who are trying to conceive, and it allows medicines in enclomiphene’s class, along with HCG, for men who want to protect fertility.
It is a poor fit when the testes themselves are failing, shown by high LH. Turning up a signal the testes cannot answer does little, and testosterone therapy is usually the route then. Men with a suspected pituitary problem need that worked up first. Men in tested sport should know that this drug class is on the World Anti-Doping Agency prohibited list.
The regulatory picture, in plain terms
You should know where enclomiphene stands before you start. Pharmacies compounding under section 503A can use an ingredient like this while the FDA decides whether to put it on its approved bulks list. The agency’s list of nominated substances, updated May 14, 2026, still shows enclomiphene citrate in Category 1, “under evaluation.”
That review has not gone its way so far. At a June 2022 advisory committee meeting, members voted 8 to 4 against adding it to the list, citing limited evidence of clinical benefit. No final decision has been published, so access depends on an interim status that could change. If it does, your provider will talk through the next option with you, and HCG or testosterone therapy would remain available.
How the clinic decides, and what your visits look like
The decision rests on your labs. Two early morning testosterone results come first, then LH and FSH, which show whether the problem sits in the brain’s signaling or in the testes. Our page on low testosterone with low LH explains how to read that pattern. Estradiol, a complete blood count and PSA round out the picture.
- Free 10-minute phone call to talk through your goals, including any plans for children.
- Same-day lab draw at the San Gabriel or Downey clinic, with results in 24 to 48 hours.
- Consultation with a PA-C or MD, who compares enclomiphene with HCG therapy and testosterone therapy for your numbers and goals, and explains the regulatory status above.
- Starting treatment: a daily pill at the dose your provider sets, with dates for your first follow-up labs.
- Follow-up labs to confirm that testosterone and LH have risen and that estradiol has not climbed too far.
| Point in treatment | What is checked | Why it matters |
|---|---|---|
| Before starting | Two morning testosterone tests, LH, FSH, estradiol, CBC, PSA | Confirms a pattern enclomiphene can work on |
| First follow-up | Testosterone, LH, estradiol | Shows whether your testes are responding |
| Around 16 weeks | Testosterone and symptoms, plus semen analysis if fertility is the goal | The phase 3 trials judged results at this point |
| Ongoing | Periodic labs at intervals your provider sets | Keeps the dose matched to your results |
Some men who started on testosterone and later decide to have children ask whether they can switch. That is a planned change, often with HCG involved, and our article on whether TRT affects fertility explains the options.
Side effects and safety
Because enclomiphene never completed FDA review, there is no approved label listing its side effects. Its parent drug is the closest guide. MedlinePlus lists for clomiphene flushing, upset stomach, breast discomfort and headache as common effects, and blurred vision, visual spots or flashes and double vision as uncommon but serious ones that need a call right away. Rising testosterone also brings rising estradiol, which is why it is measured at follow-ups.
Stop the medicine and get urgent care for sudden vision loss. For chest pain, sudden shortness of breath, one-sided weakness or face drooping, call 911.
Paying for enclomiphene therapy
Ultimate Male is a direct-pay clinic that does not bill insurance and does not publish prices. Your consultation, labs and follow-ups are paid per visit, with no membership, and the compounded medication is a separate cost your provider explains before you commit. Visits come with itemized receipts for FSA, HSA and out-of-network claims, and financing is available through CareCredit, Prosper Healthcare Lending and Afterpay.
Find out whether enclomiphene fits your labs
The right answer depends on your LH, your plans for a family and how you feel about each option. Call 626-319-5261 for the free 10-minute call or book a consultation at our San Gabriel or Downey clinic. If enclomiphene is not the right match, your provider will explain which testosterone therapy option fits instead, including HCG with testosterone for men who need replacement and want to protect fertility.
