What does a homocysteine test measure?
It measures an amino acid that your body normally breaks down quickly. MedlinePlus explains that vitamin B12, vitamin B6 and folate break homocysteine down into substances your body needs, so a high level usually means that process is short of one of those vitamins or is not working properly.
Labs disagree slightly on the cutoffs, so read your own report’s range first. A 2024 review in the International Journal of Molecular Sciences describes the common definitions this way:

Men tend to run higher than women, and levels creep up with age and smoking. That is why a result of 13 in a 60-year-old smoker means something different from the same number in a 30-year-old.
Which causes of high homocysteine can be reversed?
Most mild elevations in men have a cause that can be found and fixed. These are the common ones:
- Low vitamin B12. Absorption falls with age, and the American Family Physician review lists metformin for more than four months and acid-reducing medicines for more than a year as risk factors. Our vitamin B12 blood test page covers the gray zone where results get confusing.
- Low folate. A diet light on leafy greens, beans and fortified grains can leave folate short; see our folate blood test page.
- Reduced kidney function. MedlinePlus lists chronic kidney disease among the conditions that raise homocysteine, so the eGFR and creatinine test belongs next to it.
- Heavy drinking. Alcohol use disorder raises the risk of low B12 and folate, and the 2024 review names alcohol among the lifestyle factors that push homocysteine up.
- Underactive thyroid. MedlinePlus includes hypothyroidism among the other conditions linked to high levels.
- Two copies of an MTHFR variant. The MedlinePlus MTHFR page explains that two copies of C677T, or one each of C677T and A1298C, probably explain a high level when one is found.
What does lowering homocysteine do for your heart?
Less than the early theory promised. A Cochrane review of 15 randomized trials with 71,422 participants found no difference in heart attacks between B-vitamin supplements and placebo (7.1 percent against 6.0 percent) and no difference in death from any cause. Stroke was slightly lower with the vitamins, 4.3 percent against 5.1 percent.
The American Family Physician review reaches the same conclusion for B12 in particular: giving it to people with high homocysteine and heart disease did not lower heart attack or stroke risk. MedlinePlus adds that routine homocysteine screening for heart risk is not recommended for everyone. The practical lesson is to treat the cause, because B12 and folate shortfalls cause real problems with blood cells and, for B12, nerves, and to manage heart risk through LDL, ApoB, blood pressure and smoking. When is a homocysteine test worth doing?
It is worth doing when the result could change what happens next. That includes symptoms of a B12 or folate shortfall such as fatigue, tingling or numbness in the hands or feet and mouth sores, heavy drinking, kidney disease, or a two-copy MTHFR result. It is less useful as a stand-alone heart screen in a man who feels well.
To prepare, MedlinePlus says you may need to fast for 8 to 12 hours, and you should tell your provider about every supplement you take, especially B vitamins, because they can change the result. Do not stop a medicine on your own before the draw.
