Low HDL cholesterol in men

Short answer

For men, an HDL below 40 mg/dL is considered low. Low HDL rarely travels alone; paired with high triglycerides, it usually signals insulin resistance and excess abdominal fat. Drugs that raised HDL have not reduced heart attacks in major trials, so the aim is to treat the cause, mainly weight, activity and insulin resistance, while keeping LDL and ApoB in check.

What counts as low HDL for men?

For men, an HDL cholesterol below 40 mg/dL counts as low. MedlinePlus lists 60 mg/dL or higher as the healthiest band for men aged 20 and older, with an LDL under 100 mg/dL, a non-HDL cholesterol under 130 mg/dL and a total cholesterol under 200 mg/dL. Your report may print slightly different reference ranges depending on the lab, so read your numbers against it.

HDL is often called the “good” cholesterol because it helps carry cholesterol away from the arteries. In population studies, men with higher HDL have fewer heart attacks. The trouble is that this relationship turned out to be more of a signal than a lever, which is the key to understanding a low result.

Why does low HDL with high triglycerides point to insulin resistance?

Low HDL and high triglycerides tend to appear together because the same underlying problem drives both: insulin resistance, usually with extra fat around the middle. When insulin stops working well, the liver releases more triglyceride-rich particles, and HDL particles get remodeled and cleared faster, so one number climbs while the other falls.

The NHLBI lists low HDL and high triglycerides as two of the five features of metabolic syndrome, which it notes is also called insulin resistance syndrome. Three or more of the five (large waist, high blood pressure, high triglycerides, low HDL and high fasting glucose) make the diagnosis.

Why haven’t HDL-raising drugs lowered heart risk?

Because raising the HDL number did not translate into fewer heart attacks when it was tested directly. Several large trials raised HDL with medicines and found no benefit, or harm:

  • Niacin (AIM-HIGH). In 3,414 people with heart disease on statins, extended-release niacin raised median HDL from 35 to 42 mg/dL and lowered triglycerides, yet heart events were no lower, and the trial was stopped early for lack of benefit.
  • Niacin with laropiprant (HPS2-THRIVE). In a larger trial, HDL ran about 6 mg/dL higher with no significant drop in major vascular events, along with more serious side effects, including disturbed diabetes control and new diabetes diagnoses.
  • Torcetrapib. This drug raised HDL by 72% at 12 months but increased cardiovascular events and deaths.

Genetics points the same way. A mendelian randomization study in The Lancet found that people who inherit gene variants raising HDL do not have a lower risk of heart attack, while variants raising LDL do raise risk. The conclusion: low HDL marks a risky metabolic state, but HDL itself is not the target.

What raises or lowers HDL?

The habits that improve insulin resistance also tend to nudge HDL up. MedlinePlus notes that regular physical activity may raise HDL and that smoking lowers it. Losing abdominal fat, cutting back on refined carbohydrates and treating insulin resistance usually improve triglycerides and HDL together.

What should you focus on instead of HDL?

Focus on the markers that carry the risk and the habits that fix the cause. For heart risk, that means LDL and especially ApoB, the count of particles that can lodge in artery walls; our page on the ApoB blood test explains why it often adds more than LDL alone. For the metabolic side, it means waist size, triglycerides, fasting glucose and A1c.

A man with HDL of 34 mg/dL and triglycerides of 260 mg/dL does not need an HDL pill. He needs to know whether he is insulin resistant, and a plan to change that.

Should I take niacin to raise my HDL?
Not for heart protection. Two large trials added niacin to statins, raised HDL, and found no drop in heart attacks or strokes, and one found more serious side effects, including problems with diabetes control. Talk to your provider before starting high-dose niacin for any reason.
Is a very high HDL always good?
Not necessarily. Genetic studies show that higher HDL from some causes does not lower heart attack risk, so a high HDL does not cancel out a high LDL or ApoB.
Does smoking affect HDL?
Yes. MedlinePlus notes that smoking lowers HDL and raises LDL, so quitting improves both numbers along with your overall heart risk.
