What gets checked before a GLP-1 start?
The pre-start checks for semaglutide or tirzepatide fall into two groups: a handful of blood tests and a few history questions. Each one is there because a specific result would change the plan.

What does the A1c result change?
A1c sorts you into normal, prediabetes or diabetes, and that changes how the medicine is managed. The NIDDK puts normal below 5.7%, prediabetes at 5.7% to 6.4% and diabetes at 6.5% or above, and notes that a second test usually confirms a diabetes diagnosis.
A diabetes-range result raises two specific questions from the Wegovy label. First, combining semaglutide with insulin or an insulin secretagogue such as a sulfonylurea raises the risk of low blood sugar, so those doses may need to come down. Second, diabetic retinopathy complications have been reported in people with type 2 diabetes, so a history of diabetic eye disease calls for monitoring. Why does kidney function matter?
Kidney function matters because the common side effects of these medicines, nausea, vomiting and diarrhea, can dehydrate you, and dehydration strains the kidneys. The Wegovy label reports acute kidney injury after the drug reached the market, in some cases requiring dialysis, mostly in people who had stomach side effects, and it advises monitoring kidney function in anyone reporting those symptoms.
A baseline creatinine and eGFR lets your provider spot a drop later and know how much cushion you started with. If your filtration is already reduced, expect closer attention to fluids and to any stomach side effects as the dose rises. What do lipids and triglycerides add?
A lipid panel gives a heart-risk baseline that weight loss usually improves, and one number on it carries a safety question. The NHLBI classifies triglycerides above 500 mg/dL as very high and warns that they raise the risk of acute pancreatitis.
That matters here because acute pancreatitis has been seen with GLP-1 medicines, and the Wegovy label says to stop if it is suspected. A very high triglyceride result, or a past episode of pancreatitis, does not automatically end the conversation, but the cause has to be understood first. What if liver enzymes come back high?
Raised ALT or AST is a reason to find the cause, and for many heavier men that cause is fat in the liver. MedlinePlus lists alcohol, hepatitis and medicines among the conditions that liver tests help find, and notes that abnormal results do not always mean liver disease, since other conditions can shift some of these values.
When fatty liver is the explanation, a GLP-1 may be part of the answer rather than a barrier. The Wegovy label now includes treatment of noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH) with moderate to advanced fibrosis. Which history question outranks every lab?
The thyroid cancer question outranks every lab. Semaglutide caused thyroid C-cell tumors in rodents, and the Wegovy label makes the medicine off-limits for anyone with a personal or family history of medullary thyroid carcinoma or with multiple endocrine neoplasia type 2. MedlinePlus carries the same warning for tirzepatide and asks about any history of pancreas or kidney disease, slow stomach emptying or diabetic retinopathy.
