What does SHBG do with your testosterone?
SHBG is a carrier protein that decides how much of your testosterone sits in reserve and how much is free to act. MedlinePlus describes it as a protein made mostly in the liver that binds sex hormones, including testosterone and estrogen, and controls how much of them is active.
Testosterone in your blood travels in three forms. Some is tightly bound to SHBG and cannot reach your tissues. Some rides loosely on albumin and can be released when needed. A small share floats free. Your muscles, brain, bones and sexual function respond to the free and loosely held portions, not to the testosterone that SHBG has locked away.
Why can a normal total testosterone still leave you short?
A standard total testosterone test adds all three forms together, so it cannot tell you how much of the total is usable. As the MedlinePlus testosterone page explains, most of your testosterone is bound to SHBG and albumin, and bound hormone is harder for the body to use.
Picture two men who both test at 450 ng/dL. If one has a high SHBG, more of his 450 is locked up, and he may have the symptoms of low testosterone despite a normal-looking total. If the other has a low SHBG, more of his is free, and he may feel fine at a total that would worry him on paper. SHBG is the number that tells them apart. What pushes SHBG up or down?
Several common conditions move SHBG, and most of them are treatable or at least explainable. The Endocrine Society guideline lists conditions that raise and lower SHBG, and MedlinePlus covers the same ground for patients.

The insulin link is worth knowing about if you carry extra weight around your middle. A man carrying extra weight around the middle may have a low total testosterone partly because his SHBG is low, which is why insulin and glucose markers belong in the same conversation. How do you read an SHBG result?
There is no single cutoff for SHBG on its own. Labs report it in nanomoles per liter against their own reference range, and those ranges differ by lab method and often by age, so compare your number with the range printed on your report.
The more useful step is to combine SHBG with total testosterone and albumin to estimate free testosterone. MedlinePlus notes that providers usually look at SHBG together with total testosterone for exactly this reason. A practical way to think about it:
- High SHBG with borderline total testosterone usually calls for a free testosterone calculation before any decision.
- Low SHBG with low total testosterone may overstate the problem, so the free value matters here too.
- An SHBG far outside the range is a reason to look for the driver, such as thyroid or liver issues, rather than a reason to treat testosterone.
Does SHBG matter once you are on testosterone therapy?
It does, because SHBG affects how much of each dose ends up free. MedlinePlus lists anabolic steroids among the things that lower SHBG, so the number can shift once treatment starts. A man with naturally high SHBG may need his provider to look at free testosterone, not just total, when judging whether his levels sit in range.
SHBG also helps explain why two men on similar treatment plans can feel different.
