Avanafil (Stendra): The Fast-Onset ED Medication

Short answer

Avanafil (Stendra) is an FDA-approved PDE5 inhibitor for erectile dysfunction whose label allows the 100 and 200 mg doses to be taken as early as about 15 minutes before sex. It is more selective for PDE5 than sildenafil, and vision changes were rare in its trials. It shares the same absolute nitrate contraindication, and fewer makers mean narrower pharmacy availability.

What avanafil is and when the FDA approved it

Avanafil (Stendra) is the newest of the four PDE5 inhibitor pills for erectile dysfunction in the United States, with an initial FDA approval in 2012. Like sildenafil, tadalafil and vardenafil, it helps the penis respond to arousal; it does not cause an erection by itself.

The Stendra label lists 50 mg, 100 mg and 200 mg tablets. A generic avanafil tablet now also appears on DailyMed, but it is a far smaller market than the older pills, a point we come back to below.

How fast is it, and how does it work?

Avanafil’s main distinction is timing. The label lets men take the 100 mg or 200 mg dose as early as about 15 minutes before sexual activity, and the 50 mg dose about 30 minutes before. MedlinePlus gives the same timing and limits it to once in 24 hours. For comparison, sildenafil and vardenafil are usually taken about an hour ahead.

The mechanism is the same as the rest of the class. Sexual arousal releases nitric oxide, which raises cGMP and relaxes the blood vessels and spongy tissue of the penis. PDE5 breaks cGMP down; avanafil blocks PDE5, so the erection signal lasts longer. In the fasting state it peaks in a median of 30 to 45 minutes, and its half-life is about 5 hours, which makes it a short-acting option rather than a weekend pill.

A high-fat meal delayed the peak by a little over an hour and lowered it by 39% in testing, so a heavy dinner can cancel out the quick start.

Fewer vision and back-pain effects?

Avanafil’s lab profile is more selective than sildenafil’s. The label reports that avanafil is more than 100 times more potent at PDE5 than at PDE6, the retinal enzyme behind the blue-tinted vision some men get, while sildenafil is only about tenfold selective. Across all avanafil trials, the label notes, a single participant reported a change in color vision.

Back pain was also uncommon. In the 12-week trials it was reported by 3.2%, 2.0% and 1.1% of men on 50, 100 and 200 mg, against 1.1% on placebo, which compares with the more familiar back ache of tadalafil. Headache was the most frequent effect, rising from 5.1% at 50 mg to 10.5% at 200 mg, followed by flushing and nasal congestion.

Cost and availability: the practical limits

The biggest barrier to avanafil is not medical. As of October 2026, a DailyMed search for avanafil returns only a handful of listings, including the brand and one generic tablet, while sildenafil and tadalafil each return well over 150. Fewer manufacturers can mean patchier pharmacy stock and less price competition than the long-established generics have.

Who should not take avanafil

Avanafil carries the same absolute nitrate contraindication as every ED pill. Do not take it with nitroglycerin, isosorbide or any other nitrate, or with riociguat or vericiguat. If a nitrate becomes necessary in an emergency, the label asks for at least 12 hours after the last avanafil dose, with close monitoring. Avanafil also needs care or another choice if you:

  • take ritonavir, ketoconazole or other strong CYP3A4 inhibitors (the label says not to use it);
  • take an alpha-blocker or several blood pressure drugs, or drink more than three units of alcohol around a dose;
  • have severe kidney or liver disease, or retinitis pigmentosa, which were not studied;
  • have had a heart attack, stroke, life-threatening arrhythmia or a heart procedure in the last 6 months, groups the label says were not studied.

Sudden vision or hearing loss, chest pain, or an erection lasting longer than four hours needs 911 or an emergency room, not a callback.

Is avanafil stronger than sildenafil or tadalafil?
No head-to-head evidence shows it produces firmer erections. Its selling points are a short labeled onset and a cleaner side-effect profile in its own trials, not greater strength.
Can I take avanafil with food?
Yes. The label allows it with or without food, although a high-fat meal slowed its peak by a little over an hour in testing. If timing matters to you, a lighter meal keeps the fast onset intact.
How long after avanafil could a nitrate be given in an emergency?
The label says at least 12 hours should pass after the last dose before a nitrate is considered, and only under close medical monitoring. That window is shorter than tadalafil's 48 hours, but it is not permission to combine them.
