What does a biological age blood test measure?
It does not measure aging directly. These scores take markers that predict illness and death across large populations and convert them into an age. The best-known version, Phenotypic Age (often called PhenoAge), was developed from U.S. national survey data and then validated in a 2018 PLOS Medicine study of more than 11,000 adults followed for about 12.6 years.
If your Phenotypic Age comes out lower than your birthday age, your markers resemble those of younger people in that survey. If it comes out higher, they resemble older people’s. That is the whole idea, and it is useful as far as it goes.
Which routine markers go into PhenoAge?
Nine standard blood tests plus your chronological age. Most of them already appear on a comprehensive metabolic panel or a complete blood count, which is why the score can be calculated from ordinary lab work.

How well does it predict health?
At the population level, reasonably well. In the PLOS Medicine study, each one-year increase in Phenotypic Age beyond what birthday age predicted was linked to about 9% higher risk of death during follow-up, after adjusting for age and sex. It also tracked most cause-specific deaths, which suggests it captures general wear rather than one disease.
There is a DNA version too. DNAm PhenoAge estimates the same score from methylation patterns at 513 sites in the genome. In the 2018 paper that built it, the authors reported that the blood-marker version actually predicted illness and death better than the methylation version. A more expensive test is not automatically a sharper one.
Where do these estimates fall short?
They are snapshots built from markers that move for ordinary reasons. MedlinePlus lists bacterial and viral infections among the causes of a high CRP, and glucose depends on whether you fasted. A man tested a week after the flu can look several years older than he will a month later.
- One draw is noisy. The study’s authors named the lack of repeated measurements as their main limitation.
- It was built for groups, not individuals. A population hazard ratio does not tell you which organ is aging or what to do about it.
- Models differ. Commercial reports use different formulas, so two companies can give the same blood two different ages.
- It is not a diagnosis. A good score does not rule out disease, and a poor one does not establish it.
What should you do with the markers instead?
Read each one for what it says. A high CRP is a reason to look for inflammation and check cardiovascular risk, and our page on the hs-CRP test explains the cutoffs. A creeping glucose points toward insulin resistance. A wide red cell distribution width on a complete blood count can hint at low iron or B12. Each of those findings leads to a specific next step, which a single age number does not.
The same logic applies to your comprehensive metabolic panel: albumin, creatinine and alkaline phosphatase each have their own story, and they are more useful read in context than folded into one score.
