Is there an interaction between Cialis and testosterone?
No direct one is listed. The testosterone cypionate label names a short list of drug interactions, mainly oral blood thinners, insulin and one older anti-inflammatory drug. ED medicines are not on it. The tadalafil label focuses its warnings on nitrates, alpha-blockers, other blood pressure medicines, heavy drinking and drugs that slow its breakdown. Testosterone is not on that list either.
The two medicines also work differently. Testosterone replaces a hormone your body is not making enough of. Tadalafil blocks the PDE5 enzyme so that blood vessels in the penis stay relaxed during arousal. They do not compete for the same job.
What should you still watch when combining them?
Blood pressure, from both directions. The testosterone label warns that testosterone can raise blood pressure and asks for it to be checked periodically, especially in men who already have hypertension. Tadalafil does the opposite in a small way, and the label notes it can add to the effect of blood pressure medicines and alpha-blockers.

Why might erections not recover on testosterone alone?
Because low testosterone is rarely the only cause. The AUA erectile dysfunction guideline states plainly that testosterone therapy is not an effective stand-alone treatment for ED. It also notes that the most common underlying mechanism of ED is vascular, the same blood vessel problems that drive heart disease, and testosterone does not fix those.
What testosterone can do is support the response to other treatment. The guideline’s analysis of trials comparing a PDE5 inhibitor alone with a PDE5 inhibitor plus testosterone found the combination scored 2.69 points higher on average on the erectile function scale. The panel concluded that in symptomatic men with low testosterone, these pills are most likely to work at their full potential once testosterone levels are normalized.
When does adding Cialis to TRT make sense?
When testosterone is in range and erections still fall short. That usually looks like one of these:
- Your levels have been in range long enough for your provider to judge, and libido and energy have improved, but erections have not.
- Your ED started before your testosterone fell, which points to a separate cause.
- You have diabetes, high blood pressure or cholesterol problems that affect blood vessels.
