Why does the label require a meal?
Because Kyzatrex was studied and dosed that way. The Kyzatrex label tells patients to take it with food, starting at 200 mg in the morning and 200 mg in the evening, with the dose later adjusted from a minimum of 100 mg once daily to a maximum of 400 mg twice daily. Its main efficacy trial was run with meals, so the doses on the label assume food in your stomach.
Kyzatrex is testosterone undecanoate in a liquid-filled capsule. How much of each capsule reaches your blood depends heavily on what you eat with it, and that is what the food study measured.
How much does the fat in a meal change absorption?
A lot. The label compares testosterone exposure over 24 hours with breakfasts of different fat content against taking the capsule fasted.

Two lessons follow. First, any real meal makes a large difference compared with an empty stomach. Second, meals in the trial were not standardized: the label notes the main study was run under fed conditions regardless of meal type and still met its goal. So the practical aim is a normal meal, eaten consistently, rather than the fattiest breakfast you can find.
What counts as taking it with food?
Take the morning capsule with breakfast and the evening capsule with dinner. A black coffee or a few bites of fruit is unlikely to count as the meal the label had in mind. MedlinePlus also suggests taking the capsules at around the same times every day, which makes it easier to tie each one to a regular meal rather than remembering it on its own.
If you eat late or skip meals on some days, plan around that pattern rather than taking a capsule alone. Keeping a few capsules where you usually eat, such as a kitchen or office drawer, is a simple way to pair them with food.
Consistency matters most on lab days. The label asks for a blood test 3 to 5 hours after the morning dose, at least 7 days after starting or changing it, and its adjustment table leaves the dose alone when that result falls between 460 and 971 ng/dL. What should you do if you miss a capsule?
MedlinePlus advises taking a missed dose as soon as you remember, unless it is almost time for the next one, in which case you skip it and carry on. Never take a double dose to make up. With Kyzatrex, take a late capsule with food, so a missed breakfast dose can be caught up with a snack, or skipped if dinner is close.
If you miss doses often, tell your provider. Twice-daily timing does not suit every routine, and a schedule that keeps slipping may point to a different form of testosterone.
