How do you taper off semaglutide safely?

Short answer

With a plan made before the last full dose. Stopping semaglutide outright led to regaining about two-thirds of the lost weight within a year in the STEP 1 extension. A safer exit usually steps down through lower doses while you watch weight and appetite, and leans hard on strength training and protein. Why not just stop semaglutide cold?

Because the trials that did exactly that saw most of the weight return. In the STEP 1 trial extension, people stopped semaglutide and the lifestyle program at week 68 after losing an average of 17.3%. One year later they had regained 11.6 percentage points, about two-thirds of the loss, leaving a net loss of 5.6%. Blood pressure, A1C and other gains drifted back toward where they started.

The STEP 4 trial showed the same thing from another angle. After 20 weeks on semaglutide, people switched to placebo gained 6.9% over the next 48 weeks, while those who stayed on it lost another 7.9%.

Both trials stopped the drug outright. Neither tested a gradual taper, so a step-down plan is clinical judgment built on how the drug works, not a protocol proven in a trial. That is worth knowing going in.

What does a step-down plan look like?

It moves down through lower doses in stages, with a check at each stage. The Wegovy label has no taper schedule, but it does list the lower strengths used during the climb (0.25, 0.5, 1 and 1.7 mg) and allows 1.7 mg as a maintenance dose. Those same strengths are the natural rungs on the way down.

A plan usually defines these things up front, and your provider sets every number:

  • The rungs. Which lower doses you will use and in what order.
  • Time at each rung. Long enough to see how appetite and weight settle, usually weeks to months.
  • A weight range. A band around your current weight, so a drift outside it triggers a check-in instead of panic.
  • The off-ramp or floor. Whether the goal is to stop completely or to find the lowest dose that holds your weight.

One label detail matters if you pause entirely: after two or more missed weekly doses, the climb is restarted at a lower dose to limit stomach side effects. Coming back on is not as simple as picking up where you left off.

Why do strength training and protein matter so much here?

Because muscle is what you are trying to protect while the appetite brake comes off. A 2025 joint nutrition advisory on GLP-1 therapy notes that higher protein targets of 1.2 to 1.6 g per kilogram a day have been proposed during active weight loss, but stresses that protein alone is unlikely to preserve muscle without structured resistance training. It also notes that aerobic exercise alone does less to protect lean mass.

The CDC guidelines for adults call for at least 150 minutes of moderate activity a week plus muscle-strengthening work on two or more days. During a taper, those two strength days are the non-negotiable part.

Habits that carry the most weight once the medicine is lower:

  • Protein at every meal, planned before hunger returns
  • Two or three lifting sessions a week, progressing the load over time
  • A regular weigh-in day, the same as during treatment
  • Fewer ultraprocessed snacks and sugary drinks, which the advisory flags as problem foods

What should you expect as the dose comes down?

Expect your appetite to come back. Per the label, semaglutide has an elimination half-life of about a week and lingers in the blood for five to seven weeks after the last dose, so hunger tends to return gradually rather than overnight.
