Olympus Male vs Tadalafil: What the Custom Blend Adds

Short answer

Tadalafil (Cialis) is an FDA-approved pill that improves blood flow for erections. Olympus Male is a compounded, non-FDA-approved blend that adds PT-141 (bremelanotide), which acts on desire in the brain, and oxytocin, linked to closeness. It may suit men whose low desire or anxiety sits alongside weaker erections. For erection quality alone, tadalafil has far stronger evidence.

The verdict: who each one suits

If your erections are the main problem and desire is fine, tadalafil on its own is the stronger, simpler choice: it is FDA-approved, well studied and available as a generic. Olympus Male is worth discussing when the picture is mixed, for example when interest has faded, nerves get in the way, or you want something that targets the mental side of sex as well as blood flow. The trade-off is that two of its three ingredients have thin evidence in men, and the blend as a whole is a compounded product, not an FDA-approved one.

What each ingredient does, and how good the evidence is

Tadalafil: the blood-flow piece

Tadalafil blocks the PDE5 enzyme so the blood vessels in the penis relax more easily when you are aroused. It does not create desire, and it does nothing without sexual stimulation. The Cialis label reports better erectile function than placebo for up to 36 hours after an as-needed dose, and it also lists a once-daily option. That long window is why many men like it. If you want to compare dosing styles, our page on daily versus as-needed tadalafil walks through both.

PT-141 (bremelanotide): the desire piece

Bremelanotide works on melanocortin receptors in the brain, not on blood vessels. Its only FDA approval, under the name Vyleesi, is for premenopausal women with low sexual desire, and the Vyleesi label states plainly that it is not indicated for men. Research in men is early. In a small laboratory study of 19 men, low-dose intranasal PT-141 combined with sildenafil produced stronger erections than sildenafil alone. That is a promising signal, not proof that a troche works the same way. Our PT-141 for men page covers the research in more depth.

Oxytocin: the closeness piece

Oxytocin is the hormone tied to bonding and touch. The human evidence is the weakest of the three. A 2021 review found that studies giving men oxytocin through the nose did not confirm the sexual benefits seen in animals. One study of 29 couples found that oxytocin did not change drive, arousal or erection, but men reported more intense orgasm and more contentment afterward, with small to moderate effects. Treat the oxytocin component as a possible bonus, not the reason to choose the blend.

What “not FDA-approved” means here

Tadalafil tablets are made to a strength and purity the FDA has reviewed. A compounded blend is prepared by a licensed pharmacy for an individual patient, and the FDA states that it does not verify the safety, effectiveness or quality of compounded drugs before they are sold. The agency also says compounded drugs should be used when a patient’s needs cannot be met by an approved drug.

In practice, that means two things for you. First, a reason to choose the blend over plain tadalafil should be part of your plan, such as a desire problem tadalafil cannot address. Second, the source matters: Ultimate Male uses licensed U.S. compounding pharmacies, and you should know which one fills your medicine.

Side effects and who should not use either

Everything that applies to tadalafil applies to the blend, because the blend contains it.

  • Nitrates are a hard stop. Nitroglycerin and similar chest-pain medicines combined with tadalafil can drop blood pressure sharply. Read more on ED pills and nitrates.
  • Blood pressure cuts both ways. Tadalafil lowers blood pressure slightly, while bremelanotide raises it for a short time after each dose. The Vyleesi label lists uncontrolled hypertension and known cardiovascular disease as reasons not to use it.
  • Nausea is the big PT-141 complaint. In the Vyleesi trials, 40% of women reported nausea and about 20% reported flushing. The label also describes darkened patches of skin with frequent use.
  • Naltrexone. Bremelanotide can sharply lower blood levels of oral naltrexone, which matters if you take it for alcohol or opioid use.

Which men gain from the blend over tadalafil alone

The men most likely to benefit are those whose problem is not only mechanical. Common examples include a man whose erections respond to tadalafil but who rarely feels like initiating, or one whose anxiety in the moment undercuts a pill that otherwise works. If you are unsure which side is weaker, our explainer on low libido versus ED can help you sort it out.

Men who probably gain little are those with a pure blood-flow problem and healthy desire, and men with heart disease or hard-to-control blood pressure, for whom the PT-141 component adds risk. For a straight comparison of the brain-based and blood-flow approaches, see PT-141 versus sildenafil.

How Ultimate Male decides between them

The choice starts with what is actually wrong. At the San Gabriel or Downey clinic, your provider takes a sexual and medical history, checks your blood pressure, and reviews every medicine you take for nitrates, alpha-blockers and naltrexone. Same-day blood work looks for low testosterone, diabetes and other causes of both low desire and weak erections, with results in 24 to 48 hours.

From there, many men start with plain tadalafil because it is simpler and better proven, then move to the blend if desire remains the gap. Others go straight to Olympus Male when the history points clearly to both. Your provider sets the dose for either. Olympus Male vs Tadalafil: What the Custom Blend Adds

Still unsure? Take the free assessment

Can I take Olympus Male if I already use daily tadalafil?
Not without a review first. Olympus Male already contains tadalafil, so adding it on top of a daily tablet stacks two doses of the same drug. Your provider would adjust or replace one of them rather than combine both.
Is PT-141 the same thing as Vyleesi?
The active ingredient is the same molecule, bremelanotide. Vyleesi is the FDA-approved autoinjector for premenopausal women; the PT-141 in a compounded blend is made by a pharmacy and has not gone through FDA approval in that form.
Will Olympus Male fix low testosterone?
No. None of its three ingredients raises testosterone. If low libido comes from low testosterone, blood work shows it and treatment targets the hormone directly.
