What goes into a monthly TRT cost?
A monthly TRT cost is the sum of four parts: medication, supplies, visits and labs. Some clinics roll all four into one monthly fee, others list each separately, and Ultimate Male charges per visit with no membership. None of these models is automatically less expensive; the only fair comparison is to line up the same four parts.

Medication: the part that changes most by form
The form you choose shapes the medication line more than anything else. For injectable testosterone cypionate, the drug’s label gives a range of 50 to 400 mg every two to four weeks for men with low testosterone, so the dose your provider sets decides how long each vial lasts. Oral, pellet and add-on options follow their own logic:

Visits and labs: why the first months cost more
The first months cost more because they carry most of the monitoring. The Endocrine Society guideline recommends evaluating men 3 to 12 months after starting and then once a year, checking testosterone 3 to 6 months in, and measuring hematocrit at baseline, at 3 to 6 months and then annually. A dose change adds another check.
Spread over a year, those early visits and draws raise the average monthly cost in year one. From the second year, a stable plan usually needs fewer of them, so the monthly figure settles closer to medication plus supplies. How to compare all-in and itemized pricing
The fair way to compare is to turn every offer into a 12-month total covering the same four parts. A low monthly membership that excludes labs can end up costing more than an itemized plan, and the reverse is also true. Neither model is better by default; what matters is what you actually pay over a year for your dose and your monitoring.
- List what each price includes. Medication, supplies, visits, labs, add-ons.
- Use your real dose and form. A quote for a standard dose may not match yours.
- Count year-one labs. Ask how many draws are included and the price of the rest.
- Ask about dose changes. Some plans charge for the review and recheck that follow.
- Get it in writing. If you are paying directly, CMS medical bill rights say providers must usually give a good faith estimate when you ask.
