Ferritin blood test for men

Short answer

Ferritin is the protein that stores iron, and its blood level tracks your iron reserves. MedlinePlus lists about 30 to 400 ng/mL as normal for men. A low ferritin in an adult man is treated as a sign of possible blood loss, usually from the gut, until proven otherwise, and low iron stores can cause fatigue, restless legs and hair shedding.

What ferritin measures

Ferritin is the protein that holds iron in storage, mostly in the liver, and the small amount in your blood mirrors how full those stores are. MedlinePlus explains that serum ferritin correlates with total body iron, which makes it a useful single test of whether your reserves are adequate.

Iron itself is what your body uses to build hemoglobin. Ferritin shows the savings account behind it, which is why it can fall well before hemoglobin does.

Ferritin ranges for men

MedlinePlus lists a normal range of 30 to 400 ng/mL for men, and labs set their own ranges. The same source adds a point that matters more than the cutoff: the lower the ferritin, even inside the normal range, the more likely stores are not enough.

The 45 ng/mL line comes from the American Gastroenterological Association’s 2020 guideline, which recommends that cutoff over the older 15 ng/mL when deciding whether anemia is due to iron deficiency. Ferritin also rises with inflammation, so a normal value during illness can hide low stores; a transferrin saturation test helps in that case.

Why low ferritin in a man means looking for blood loss

Adult men have no regular monthly iron loss, so a low ferritin needs an explanation beyond diet. MedlinePlus lists bleeding in the esophagus, stomach or intestines, poor absorption of iron and a diet very low in iron among the causes, and in men the first of these is the one that cannot be missed.

The AGA guideline recommends both an upper endoscopy and a colonoscopy for men with iron deficiency anemia, even without stomach symptoms, because slow bleeding from ulcers, polyps or bowel cancer often causes none. Regular use of aspirin or anti-inflammatory pain relievers, frequent blood donation and conditions that block absorption, such as celiac disease, are other common contributors. How low iron stores show up

Low iron stores can cause symptoms before anemia appears. MedlinePlus lists iron deficiency without anemia as a recognized cause of fatigue, and men often describe it as running out of energy by mid-afternoon or struggling with workouts that used to feel easy.

Restless legs are another clue. MedlinePlus’s ferritin test page notes that restless legs syndrome, a creeping or burning urge to move the legs at night, may be caused by a lack of iron. Hair can thin too: the American Academy of Dermatology lists too little iron among the causes of noticeable hair loss, with regrowth once the body gets enough. Signs of bleeding that need urgent care

Some findings mean the bleeding is active and should not wait for a lab appointment. Go to an emergency room or call 911 for black or tarry stools, vomiting blood or material that looks like coffee grounds, fainting, a racing heartbeat at rest or chest pain.

Ferritin and testosterone therapy

Ferritin and TRT meet in two places. Testosterone raises red cell production, which uses iron, and men whose hematocrit climbs on treatment are sometimes managed with blood donation or therapeutic phlebotomy, which removes iron with every unit. How to prepare for a ferritin test

MedlinePlus notes that you may be asked to fast for up to 12 hours and that the test is usually done in the morning. Tell your provider about iron supplements, multivitamins with iron and any recent infection, since each can shift the result.

Can I just take iron if my ferritin is low?
Iron supplements can refill stores, but in a man they should not replace finding out why the iron ran low. Starting iron first can also make a later workup harder to read, so talk to your provider before you begin.
Does regular blood donation lower ferritin?
Yes. Each donation removes iron along with red cells, and frequent donors can run low. Tell your provider how often you donate so the result is read in context.
Is a ferritin of 40 ng/mL low?
It sits inside many lab ranges, but MedlinePlus notes that the lower the value, even within normal, the more likely iron stores are short. In someone with anemia, a gastroenterology guideline treats values under 45 ng/mL as consistent with iron deficiency.
