Free T4 test

Short answer

A free T4 test measures the unbound thyroxine your thyroid releases, and it confirms or refines what a TSH result suggests. High TSH with low free T4 points to the thyroid gland itself. Low free T4 with a TSH that is low or normal points upstream to the pituitary. In a man who also has low testosterone with low or normal LH, that combination calls for a pituitary workup.

What does free T4 add to a TSH result?

TSH is the signal and free T4 is the response. TSH comes from the pituitary gland and tells the thyroid how hard to work, while free T4 is the unbound thyroid hormone that actually reaches your tissues. The American Thyroid Association recommends TSH as the first test of thyroid function and says free T4 more accurately reflects how the gland is working when checked with a TSH.

Most of the time, TSH alone is enough to screen. Free T4 earns its place when TSH is abnormal, when symptoms do not match a normal TSH, or when there is any reason to doubt the pituitary is sending an accurate signal. That last situation is the one men with low testosterone need to know about.

What is a normal free T4, and what moves it?

The MedlinePlus encyclopedia gives a typical range of 0.8 to 1.9 ng/dL (10.3 to 24.5 pmol/L), and notes that ranges vary among labs. It also notes that the range is based on a large population and is not necessarily normal for every individual.

Several things can push free T4 around without thyroid disease. MedlinePlus lists biotin supplements, pregnancy, estrogen, liver and kidney disease, and severe body-wide illness as factors that affect results, and lists malnutrition or fasting and severe acute illness among causes of a low value. Biotin is the easiest to fix: the American Thyroid Association advises stopping it for 2 days before thyroid blood work.

How do TSH and free T4 read together?

The pair tells you where a problem starts. These patterns come from the American Thyroid Association:

TSH Free T4 Where the problem likely starts
High Low The thyroid gland (primary hypothyroidism)
Low High The thyroid making too much (hyperthyroidism)
Low Low The pituitary gland
Low or normal, when it should be high Low The pituitary gland
High Normal Early or mild thyroid underactivity

The fourth row is the subtle one. A healthy pituitary responds to low thyroid hormone by pushing TSH up. The ATA’s hypothyroidism page notes that if the pituitary is damaged, the TSH test may not help, because the signal itself is weak. A “normal” TSH sitting next to a low free T4 is therefore not reassuring. The last row has its own page on high TSH with normal free T4.

Why does a low free T4 matter if your testosterone is low too?

Because the pituitary controls both. The same gland that makes TSH also makes LH and FSH, which drive testosterone production in the testes. When a man has low testosterone with low or normal LH, the problem sits upstream of the testes, a pattern called secondary hypogonadism. Add a low free T4 with a low or normal TSH, and two pituitary outputs are falling short at once. That raises the question of a pituitary problem, such as a pituitary tumor or damage from surgery or injury. The Endocrine Society guideline recommends measuring LH and FSH to separate testicular from pituitary causes, and further evaluation to find the cause when the problem is pituitary or hypothalamic. The Endocrine Society patient guide notes that this can include more blood tests and sometimes a pituitary MRI.

How to prepare for a free T4 test

Preparation is simple. List every medicine and supplement, pause biotin for 2 days if your provider agrees, and mention any recent severe illness or very low-calorie dieting. Fasting is only needed if other tests on the same draw require it. Do not stop a thyroid medicine before testing unless the clinician managing it tells you to.

Can my free T4 be normal while I still have thyroid symptoms?
Yes. The MedlinePlus encyclopedia notes that the normal range is based on a large population and is not necessarily normal for every individual, so symptoms can occur within range. TSH helps decide whether thyroid disease is the cause.
Does testosterone therapy change free T4?
Testosterone is not on the MedlinePlus list of common free T4 interferences, which focuses on biotin, pregnancy, estrogen, liver problems and severe illness. Mention it anyway so your provider can read the full picture.
