How does semaglutide change your blood work?

Short answer

Semaglutide usually moves blood work in a healthier direction. A1c falls, triglycerides drop and liver enzymes such as ALT often ease as weight comes off. Two things need watching. Vomiting or diarrhea can dehydrate you and strain the kidneys, so creatinine and eGFR matter during dose increases, and lipase is checked if belly pain suggests pancreatitis.

Which blood markers usually improve on semaglutide?

Blood sugar, triglycerides and liver enzymes are the markers most likely to move the right way. The changes come partly from the medicine’s direct effect on blood sugar and partly from the weight loss it brings.

The FDA label for Wegovy (semaglutide 2.4 mg) reports these changes at week 68 in its weight trials:

All figures are from the Wegovy product label. Men who start with a normal A1c should expect a smaller drop, since there is less room to fall.

Liver enzymes often follow. In August 2025 the FDA approved Wegovy under accelerated approval to treat MASH, a fatty liver disease, in adults with moderate-to-advanced scarring, and the label reports a trend toward larger ALT and AST reductions than placebo in that trial. Which results need watching?

Kidney function and lipase are the two to keep an eye on. Neither is a reason to avoid the medicine for most men, but both explain why follow-up labs exist.

Kidney function. Nausea, vomiting and diarrhea are the common side effects, and they can dehydrate you. The label reports postmarketing cases of acute kidney injury, some needing dialysis, and advises monitoring kidney function in patients whose side effects could cause fluid loss, with extra attention during dose increases. Lipase. Lipase is an enzyme from the pancreas. In the weight trials it rose an average of 39% on semaglutide and not on placebo, so a mildly high lipase without symptoms is common and hard to read on its own. What matters is pancreatitis: the label says to watch for its signs and stop the drug if it is suspected. That is why lipase is usually checked when symptoms appear rather than on a fixed schedule.

Gallbladder. The label also links semaglutide to more gallstones and gallbladder inflammation, sometimes showing up as changes in liver tests. Our answer on GLP-1 gallbladder problems covers the warning signs.

What does a typical follow-up lab schedule look like?

Your provider sets the schedule, but it usually follows the way each marker behaves.

  • Before starting: A1c, kidney and liver function, a lipid panel and a blood count, as described on our page about blood work before semaglutide or tirzepatide
  • During dose increases: kidney function sooner if vomiting or diarrhea lasts more than a day or two, or if you cannot keep fluids down
  • About three months in: A1c, since MedlinePlus describes it as reflecting average glucose over roughly the past three months, plus lipids and liver enzymes
  • Any time: lipase and urgent evaluation for severe, lasting belly pain
  • Ongoing: at intervals your provider sets, often lined up with dose changes and weight check-ins

Men who also have diabetes usually test A1c more often; MedlinePlus advises at least twice a year for people with diabetes.

When are symptoms urgent rather than a lab question?

Severe pain in the upper belly that may spread to the back, with or without vomiting, can mean pancreatitis. MedlinePlus lists this among serious side effects needing prompt care. Go to an emergency room rather than waiting for a scheduled blood test. Signs of a serious allergic reaction, such as swelling of the face or throat or trouble breathing, mean calling 911.

Will semaglutide lower my cholesterol?
Modestly. In the Wegovy label's main weight trial in adults without diabetes, LDL fell 2.5% on semaglutide versus a 1.3% rise on placebo, and HDL rose 5.2% versus 1.4%. Triglycerides moved much more than LDL.
Do I need blood work before starting semaglutide?
Yes. A baseline A1c, kidney and liver function and a lipid panel give your provider something to compare against and can show reasons to adjust the plan before you start.
Can semaglutide cause low blood sugar?
On its own it rarely does, but the Wegovy label notes a higher risk of low blood sugar when it is used with insulin, and advises considering a lower dose of insulin or insulin secretagogues when semaglutide starts.
