Free testosterone test

Short answer

A free testosterone test measures the small fraction of testosterone that is not bound to proteins. It changes the decision mainly when total testosterone sits near the lower limit of normal or when SHBG is unusually high or low. The result is only reliable when measured by equilibrium dialysis or calculated from total testosterone, SHBG and albumin. Direct analog immunoassays are inaccurate.

What a free testosterone test measures

A free testosterone test measures the testosterone in your blood that is not attached to a carrier protein. Most of your testosterone travels bound, tightly to sex hormone-binding globulin (SHBG) and loosely to albumin. Only a small unbound fraction moves freely into tissues, and that is the share this test tries to capture.

MedlinePlus describes three kinds of testosterone testing: total, free and bioavailable. Total testosterone is the standard first test. Free testosterone is a second-line test, ordered when the total alone could give the wrong answer.

When does free testosterone change the call?

Free testosterone changes the decision in two situations: when your total sits near the lower limit of normal, and when something has pushed SHBG unusually high or low. The Endocrine Society guideline recommends measuring it in exactly those cases.

The logic is simple. If SHBG is high, more testosterone is locked away, so a total in the normal range can hide a low usable fraction. The guideline notes that conditions which raise SHBG can lift total testosterone well above 400 ng/dL while free testosterone stays low. If SHBG is low, the reverse happens: the total can drop below range while free testosterone is fine.

The American Urological Association does not recommend free testosterone as the primary diagnostic test, so it supplements the total rather than replacing it. Why the method on your report matters

The method behind a free testosterone result matters as much as the number. Three approaches are in common use, and they do not agree with each other.

  • Equilibrium dialysis. The reference method. Serum sits against a membrane that only unbound testosterone can cross, and that fraction is measured. The Endocrine Society names it as the method to use, performed under standardized conditions.
  • Calculated free testosterone. When dialysis is not available, the guideline accepts an estimate calculated from an accurate total testosterone, SHBG and albumin. Formulas built on an allosteric model come close to dialysis results. Our free testosterone calculator explains how the math works.
  • Direct analog immunoassay. A fast, inexpensive kit that estimates free testosterone directly. The guideline states plainly that these assays are inaccurate and should not be used.

Because analog assays are unreliable, a result from one can label a man low when he is not, or miss a real deficiency. Check your report for the method name, which is often printed beside the result as dialysis, calculated or direct. If it says direct or analog, ask for a repeat by a reliable method before anyone uses it to make a treatment decision.

What counts as low free testosterone?

There is no single agreed cutoff. The Endocrine Society notes that a harmonized reference range for free testosterone has not been established and encourages each laboratory to set its own. That is different from total testosterone, where a harmonized adult range of 264 to 916 ng/dL exists.

For context, the guideline cites European Male Aging Study data in which sexual symptoms clustered with free testosterone below about 64 pg/mL alongside a total below 320 ng/dL. That is an association from a population study, not a diagnostic line. Units also differ between labs (pg/mL, pmol/L or ng/dL), so compare your result only with the range printed on your own report. How to prepare for the draw

Free testosterone is drawn with total testosterone and SHBG, so the same preparation applies to all three:

  • Book an early-morning draw and come fasting, because food and glucose can suppress testosterone.
  • Avoid testing during or just after an acute illness, which can give a falsely low result.
  • Mention supplements such as biotin, which the guideline flags as a source of interference in some immunoassays.
  • Keep your usual sleep and training routine the day before.

Is bioavailable testosterone the same as free testosterone?
Not quite. Bioavailable testosterone adds the loosely albumin-bound fraction to the free fraction, so it reads higher. Both try to answer the same question, how much testosterone your tissues can readily use.
Can a normal free testosterone change things if my total is low?
It can. When SHBG is low, as often happens with obesity or type 2 diabetes, total testosterone can fall below range while free testosterone stays normal, and that is handled differently from true deficiency.
Does a free testosterone test need a morning draw too?
Yes. It is drawn with total testosterone and SHBG, so the same early-morning, fasting timing applies to all three.
