Can you take semaglutide and testosterone together?

Short answer

Yes, the two can be used together under one plan, but the order matters. For men whose low testosterone is tied to excess weight, the Endocrine Society calls weight loss the first-line treatment, and losing weight can raise testosterone on its own. When both are used, hematocrit, estradiol, blood pressure, blood sugar and lean mass are watched, and your provider sets each dose.

Can losing weight raise testosterone enough to skip TRT?

For some men, yes. A meta-analysis of 24 studies found that both low-calorie diets and bariatric surgery raised total testosterone in men with obesity, with bigger rises in men who lost more weight and in younger men without diabetes. The same analysis found that losing weight lowered estradiol and lifted LH and FSH, the pituitary signals to the testes.

Guidelines build on that. The Endocrine Society’s 2026 statement says that when low testosterone is attributed to obesity, with a BMI over 27 and no other cause found, weight loss is typically the first-line treatment.

When does running both make sense?

Using both at once is reasonable in a few situations:

  • Low testosterone with a cause weight loss will not fix, such as a testicular or pituitary problem. The same Endocrine Society statement says TRT has clear benefits for those men.
  • Men already on TRT who also meet the criteria for medical weight loss.
  • Persistently low testosterone with symptoms after meaningful weight loss, confirmed on repeat morning tests.

Muscle is part of the conversation. A 2025 joint nutrition advisory reports that in the STEP 1 trial, about 38% of the weight lost on semaglutide was lean mass, and recommends strength training at least three times a week plus enough protein. The AUA notes that testosterone therapy increases lean mass in men with low testosterone, but it is not a substitute for training, and it is not a reason to start TRT without a diagnosis. Which labs are watched on the combination?

Each medicine nudges different numbers, so the monitoring overlaps.

The dehydration point is easy to miss. MedlinePlus calls dehydration the most common cause of a high hematocrit, and the Wegovy label asks clinicians to watch kidney function in patients whose stomach side effects could lead to fluid loss. Does semaglutide change how oral testosterone works?

It might. The Wegovy label notes that semaglutide slows stomach emptying and can affect the absorption of oral medicines taken at the same time, and advises monitoring their effects. Kyzatrex, the oral testosterone capsule, is taken with food, so smaller meals or nausea on semaglutide are worth mentioning.

If excess weight is the likely cause of low testosterone, weight loss usually comes first and testosterone is rechecked afterward. If the cause is a testicular or pituitary problem, the order matters less, and your provider decides based on your labs.
Does semaglutide lower testosterone?
The weight loss it produces tends to push testosterone the other way. In studies of men with obesity, testosterone rose as weight came off, more so with greater weight loss, so your provider rechecks levels rather than assuming.
Can I use tirzepatide instead of semaglutide with TRT?
The same logic applies. Weight loss comes first when weight is the cause, with shared monitoring if both are used, and your provider weighs which medicine fits your health history.
