How long does Cialis last?

Short answer

One dose of tadalafil (Cialis) can improve erections for up to 36 hours. That window comes from its long half-life of about 17.5 hours, which means roughly half a dose is still in your blood the next day and most of it is gone after three to four days. The long tail is also why emergency nitrates must wait at least 48 hours after your last dose.

What does the 36-hour window mean?

It is the stretch of time in which arousal can lead to an erection, not the length of an erection. The Cialis label reports that one as-needed dose improved erectile function compared with placebo for up to 36 hours. Two trials tested men specifically at 24 and 36 hours after a dose, and in the larger one, tadalafil beat placebo at both time points.

What does a 17.5-hour half-life mean in practice?

That the drug fades slowly over several days. The label gives a mean half-life of 17.5 hours in healthy men, meaning the amount in your blood halves about every 17.5 hours once absorption is done. Counting forward from the peak gives a rough picture:

These are averages from a simple half-life model, not a forecast for you. The label notes that men aged 65 and over clear it more slowly, and kidney or liver disease changes the dosing because the drug lingers longer.

The 36-hour window sits inside that curve. The trials did not test beyond 36 hours, but the table shows why some drug is still present for a couple of days after that.

Why does this matter for the 48-hour nitrate rule?

Because a nitrate given while tadalafil is still active can drop blood pressure dangerously. The label says that if a nitrate is medically necessary in a life-threatening situation, at least 48 hours should pass after the last dose, and even then only with close monitoring. The American Heart Association statement, summarized in American Family Physician, gives the same 48 hours for tadalafil and 24 hours for sildenafil.

How is daily Cialis different?

Daily dosing uses a lower amount to keep a steady level instead of a single peak. The label says steady-state levels are reached within five days of once-daily dosing, at about 1.6 times the level after a single dose. The practical effect is that you stop thinking about timing at all.

Do the side effects last as long?

Some of them can. MedlinePlus lists headache, indigestion, flushing and back or muscle pain among the common ones. The label describes the back and muscle aches as typically starting 12 to 24 hours after a dose and fading within 48 hours, which fits the drug’s slow exit. Does Cialis last longer in older men?
A little. The label reports that healthy men aged 65 and over cleared tadalafil more slowly, giving about 25% higher exposure, though no dose change is needed for age alone. Kidney and liver problems can slow clearance more and do change the dosing.
Can I take another dose the next day if the first one is still working?
The label allows no more than one dose a day for most men, but because each dose lingers, back-to-back days build up a higher level. Ask your provider whether daily low-dose tadalafil suits you better than repeated on-demand doses.
Does a meal make it last a shorter time?
