What does the four-to-five-hour window mean?
It is the time during which arousal can lead to an erection, counted from when you take the pill. The Viagra label says the usual dose is taken about an hour before sex but may be taken anywhere from 30 minutes to 4 hours beforehand. In a study measuring erections with a monitoring device, the effect lasted up to 4 hours, though it was weaker at 4 hours than at 2.
Two pieces of the label explain why the window is a little longer than four hours:

So the effect tapers rather than switching off at a set hour. On average, the strongest stretch falls between about one and four hours after the dose.
Does Viagra cause a four-hour erection?
No. Sildenafil has no effect without sexual stimulation, according to the label. It helps blood flow into the penis when you are aroused, and once arousal ends or you climax, the erection goes down in the normal way.

Can you have more than one erection on one pill?
Possibly, yes, as long as you are still within the window and aroused again. In a small study of 20 healthy men published in Human Reproduction, sildenafil shortened the recovery time between ejaculation and the next erection from about 11 minutes on placebo to under 3 minutes, with continued erotic stimulation. Those men did not have ED, so results in men who do will vary, but the pill does not stop working after the first erection.
What shortens or stretches the window?
Food, your body and your other medicines. Taking sildenafil after a heavy, high-fat meal delays the start, and our answer on taking Viagra on an empty stomach covers that. The label also notes that men over 65, men with liver problems or severe kidney disease, and men taking certain drugs that slow its breakdown have higher blood levels, and the label suggests considering a 25 mg starting dose for them.
MedlinePlus adds that sildenafil should usually not be taken more than once every 24 hours, and that it does not increase sexual desire. If desire rather than firmness is the issue, a longer window will not help.
Can you take a second pill when the first wears off?
No. The label sets a maximum of one dose a day. A second pill the same day raises the risk of side effects such as headache, flushing, dizziness and low blood pressure without a proven benefit. If one dose regularly runs out before you are ready, the fix is a different plan, not a second dose.
