Why am I so tired on semaglutide?

Short answer

Most often because you are eating and drinking far less than your body needs. Fatigue affected 11% of people on semaglutide 2.4 mg versus 5% on placebo in the weight trials. The usual drivers are too few calories, too little protein, dehydration and lost electrolytes. If energy does not recover with better intake, labs such as a metabolic panel, CBC, ferritin, B12 and TSH can find other causes.

How common is fatigue on semaglutide?

Common enough to be on the label. In the three main weight trials pooled in the Wegovy label, fatigue or weakness was reported by 11% of people on semaglutide 2.4 mg and 5% on placebo. In the trials of the higher 7.2 mg dose, it was 11% at 7.2 mg, 9% at 2.4 mg and 5% on placebo.

That means most people on semaglutide do not feel unusually tired. When you do, it usually points to something fixable in how you are eating and drinking, rather than to the drug acting on your energy directly.

Are you simply not eating enough?

That is the first thing to check. Semaglutide turns down hunger so effectively that some men drift to one small meal a day without noticing. A 2025 nutrition advisory on GLP-1 therapy warns that reduced appetite can lead to low protein intake and muscle loss, and lists fatigue beyond what you would expect as one sign of a true nutrient shortfall, along with heavy hair shedding, weakness and poor wound healing.

Signs that under-eating is the cause:

  • You often go most of the day without a real meal
  • Your weight is falling much faster than planned, covered in losing weight too fast on a GLP-1
  • Workouts feel flat and strength is sliding
  • You feel better on days you manage a full meal

Could dehydration or electrolytes be the cause?

Yes, especially in the early months. Many people drink less when they eat less, and vomiting or diarrhea during the dose climb drains fluid and salts. MedlinePlus lists feeling tired, dizziness, dark urine, dry mouth and urinating less than usual among the symptoms of dehydration in adults. The label adds that dehydration from stomach side effects has caused kidney injury in some people on semaglutide.

Heat makes it worse, which matters for men who work outdoors or train hard; see GLP-1 treatment during a heat wave. Steady sips through the day beat large amounts at once when your stomach feels full.

Get medical help right away if tiredness comes with confusion, fainting, very little urine or a racing heartbeat. If you take insulin or a sulfonylurea, drowsiness with shakiness or sweating can mean low blood sugar, which needs treating immediately.

Which labs are worth checking if energy does not recover?

If better eating and drinking do not help, blood work can rule other causes in or out.

Morning testosterone Low levels in men with obesity can add to low energy

Does the fatigue go away on its own?
Often it eases as the dose stabilizes and eating settles into a routine. Fatigue that lasts beyond the dose climb, or that comes with dizziness or a racing heart, deserves a call and probably labs.
Could low testosterone be part of it?
It can be, especially in men with obesity, where low testosterone is common and tends to rise as weight comes off. Your provider can check it if symptoms such as low drive or low morning energy fit.
