Why did shortage-era compounded pricing end?
It ended because the shortage that allowed it ended. While semaglutide was on the FDA’s drug shortage list, compounding pharmacies could legally make copies of it at scale, which is what supported low monthly prices. Once supply recovered, that permission ran out.

When is 503A compounding still allowed?
A state-licensed 503A pharmacy can still compound semaglutide for one identified patient, on that patient’s individual order, under narrow conditions. The key test is whether the product is “essentially a copy” of an approved drug: same active ingredient, a similar or easily substituted strength, and the same route.
A copy is acceptable only when the clinician determines and documents that the compounded version contains a change that makes a significant difference for that specific patient. The FDA has also said it does not currently intend to act against a pharmacy that fills four or fewer orders of a copy in a calendar month. Two more limits matter:
- Added ingredients do not create an exemption on their own. The FDA’s example is semaglutide combined with vitamin B12, which may still count as a copy.
- Salt forms are out. The FDA’s page on unapproved GLP-1 drugs states that semaglutide sodium and semaglutide acetate should not be used to compound semaglutide.

What does that mean for the price?
Individualized compounding is a low-volume medical exception, and that changes its cost. These are the pieces that drive it:

- What is the documented clinical reason the approved product will not work for me?
- Which licensed pharmacy makes it, and is it a 503A pharmacy?
- Is the active ingredient semaglutide base, not a salt form?
- How is my dose measured, and who shows me how to draw it up?
- What labs and follow-up come with it?
