The FDA’s page on unapproved GLP-1 drugs states that compounded drugs are not FDA-approved, that it has received reports of dosing errors with compounded semaglutide, and that salt forms such as semaglutide sodium are different active ingredients from the approved drugs, with no known lawful basis for compounding them. It advises filling GLP-1 medicines only at a state-licensed pharmacy.

The weekly dose-escalation steps

The dose rises slowly so your stomach can adjust. The Wegovy label, revised in June 2026, sets these weekly injection steps:

Check-ins, and who should not take semaglutide

Semaglutide is not for men with a personal or family history of medullary thyroid carcinoma or multiple endocrine neoplasia type 2, which the label lists as contraindications after thyroid tumors were seen in rodents. The label does not recommend it with severe gastroparesis. MedlinePlus also asks you to tell your doctor about past pancreatitis, gallbladder disease, kidney disease or diabetic eye disease.

Managing nausea and other side effects

Stomach side effects are common, especially after each dose increase. In the adult weight loss trials on the label, gastrointestinal reactions occurred in 73% of people on Wegovy injections against 47% on placebo, and were severe in 4.1%. Practical steps that help:

  • Eat smaller meals and stop at the first sign of fullness
  • Go easy on fatty, fried and very sweet foods, especially the day of and after your dose
  • Sip water through the day; dehydration makes nausea worse
  • Tell your provider early, because slowing the dose steps is often the fix
