Signs You May Need Testosterone Replacement Therapy

Short answer

Common signs include a lower sex drive, fewer morning erections, erectile changes, fatigue, loss of muscle and strength, more body fat, low mood and loss of body hair. Symptoms alone cannot confirm low testosterone. Guidelines require at least two low early-morning blood tests plus fitting symptoms, and other causes such as poor sleep, weight and medicines are checked first.

What are the signs and symptoms of low testosterone?

Low testosterone tends to show up as a cluster of changes in energy, sexual function, mood and body composition. For men, feeling “off” does not always mean you are simply stressed or getting older. If energy is low, sexual function has changed and motivation is slipping, there may be a hormonal reason worth checking.

Low testosterone can affect how you feel, think and perform throughout the day. Symptoms often appear gradually, which is why many men chalk them up to busy schedules or age. If several of these sound familiar, it may be time to look into testosterone replacement therapy:

  • low energy,
  • reduced sex drive,
  • erectile changes or fewer morning erections,
  • brain fog,
  • irritability,
  • uneven or low mood,
  • decreased strength and muscle mass,
  • slower recovery after workouts or injury,
  • increased belly fat,
  • poor sleep,
  • loss of body hair.

The Endocrine Society’s patient guide lists many of the same changes, including reduced sex drive, erectile problems, low energy, reduced muscle mass and mood changes. Symptoms start the conversation, labs decide it

None of these signs proves low testosterone on its own. The Endocrine Society calls for at least two early-morning blood tests, drawn between about 7 and 10 a.m., that both come back low, together with symptoms that fit. The AUA guideline treats a total testosterone below 300 ng/dL as a reasonable cutoff.

Am I too young for testosterone replacement therapy?

Not necessarily, because age is only one of many causes of low testosterone. Contrary to what you may have heard, the link between age and low testosterone is not always linear or predictable. Excessive exercise, high stress, poor sleep, weight changes, certain medicines and some medical conditions can all cause or contribute to lower male hormone levels at almost any age.

Some changes show up within weeks, while others take months. Timing varies with the treatment type, dose, starting level and habits such as training, sleep and nutrition. A review in the European Journal of Endocrinology pooled published studies to map the usual pattern:

Can my testosterone be normal even though I have these symptoms?
Yes. Fatigue, low libido and brain fog have many causes, including sleep apnea, thyroid problems, depression and medicines. A normal result points the search elsewhere, and sometimes free testosterone and SHBG are checked when the total looks borderline.
Do I need to fast before a testosterone test?
Guidelines prefer an early-morning draw, and the Endocrine Society recommends a fasting sample, because eating beforehand can lower the reading. Your clinic will tell you how to prepare.
