Low T after 35: what TRT can and cannot improve

Many hard-working men live with symptoms of low testosterone (low T) for years before getting tested. Quick refresher: what TRT can improve
The American Urological Association guideline tells men with confirmed testosterone deficiency that treatment may improve several things, and it is honest that results vary:
- Libido, erections and overall sexual function
- Lean muscle, strength and body composition
- Mood and mild depressive symptoms
- Bone density and anemia linked to low testosterone
- Daytime energy for physically demanding work, though this is the least predictable benefit
TRT is not a sleep treatment, and sleep apnea is one of the conditions to raise before starting testosterone, so sleep problems get their own evaluation.

Comparing your options one by one
Each option below follows the same pattern: who it suits, how it works, its strengths and what to weigh. Four of them are on Ultimate Male’s menu. Two, gel and enclomiphene, are included because men ask about them, and we are clear about where they stand.
1) Injectable testosterone
Suits: men who want flexible, precise dosing and do not mind a small needle.
How it works: testosterone cypionate goes into muscle or under the skin on a set schedule, often weekly, either in clinic or at home after we show you how.
Strengths: easy to fine-tune, with a long track record. More about it on our testosterone injections page.
Considerations: levels rise after each dose and drift down before the next, so timing of lab draws matters. Hematocrit needs regular checks.
2a) Oral option: Kyzatrex (testosterone undecanoate)
Suits: men who want oral TRT without injections or gels.
How it works: capsules absorbed through the lymphatic system, taken with food, as MedlinePlus directs for oral testosterone undecanoate.
Strengths: convenient and needle-free, with steady support when the dose is set correctly. See our Kyzatrex page.
Considerations: you need to take it with meals, and oral testosterone can raise blood pressure, so blood pressure and lipids are monitored.
2b) Oral option: enclomiphene
Suits: men who prioritize fertility or want to stimulate their own testosterone production.
How it works: a selective estrogen receptor modulator (SERM) that raises LH and FSH, signaling the testicles to make more testosterone.
Strengths: it does not suppress sperm production the way outside testosterone can.
Considerations: enclomiphene is not FDA-approved, it works only when the testes can respond, and it needs regular labs. It is not part of Ultimate Male’s current treatment menu. 3) hCG (human chorionic gonadotropin)
Suits: men who want to preserve testicular function, fertility and testicular size during TRT or instead of it.
How it works: hCG mimics LH and stimulates testosterone production inside the testes. The Pregnyl label lists hypogonadotropic hypogonadism in men among its approved uses, with injections given two to three times a week.
Strengths: fertility-friendly, and it can be used alone or added to TRT. Learn more about hCG therapy for men.
Considerations: several injections a week, dose adjustments based on labs, and monitoring of estradiol.
4) Transdermal gel
Suits: men who prefer daily, needle-free dosing with steady levels.
How it works: applied every day to clean, dry skin.
Strengths: simple and easy to pause or adjust.
Considerations: it is a daily habit, and some men absorb too little. Gel can also transfer to partners and children through skin contact, which is why MedlinePlus advises covering the site and washing your hands. Ultimate Male does not currently offer gel; men who want a needle-free option usually discuss Kyzatrex.
5) Pellet insertion
Suits: men who want long-acting convenience without weekly dosing.
How it works: small testosterone pellets are placed under the skin in a quick office procedure and release testosterone steadily. Ultimate Male uses Pellecome pellets. The FDA-approved pellet label says the effect usually lasts three to four months, sometimes up to six.
Strengths: no weekly routine and steady levels once settled. See our testosterone pellets page.
Considerations: it is a minor procedure with a small risk of infection or a pellet working its way out, and the dose is locked in until the pellets dissolve.

Safety and monitoring (our non-negotiables)
Every option on this page needs follow-up, whatever form you choose:
- Baseline and follow-up labs: total and free testosterone, SHBG, estradiol, CBC and hematocrit, CMP and lipids, plus PSA, LH, FSH and prolactin as indicated.
- Dose adjustments: based on symptoms and labs together, not numbers alone.
- Risk checks: blood pressure, hematocrit, estradiol balance, skin transfer for anyone using gel elsewhere, and your individual heart and metabolic risk.
- Add-ons when needed: hCG for fertility or testicular support, estrogen management when clinically indicated, and practical guidance on sleep, lifting and protein.
Fertility deserves its own mention. The American Society for Reproductive Medicine notes that testosterone treatment usually leads to low or absent sperm counts. If children are in your plans, that changes which option comes first.
