Which one fits you
Tadalafil suits men who want flexibility: one dose that covers a long stretch, or a small daily dose so that timing barely matters. It also helps urinary symptoms from an enlarged prostate. Vardenafil suits men who prefer a shorter-acting pill, who have done well on it before, or who like a tablet that dissolves on the tongue. It is a poor fit for men with long QT syndrome or those taking certain heart rhythm drugs. Both are FDA-approved PDE5 inhibitors, and both need sexual stimulation to work.

How each one works, and why the window differs
Both drugs block PDE5, the enzyme that ends the blood-flow signal in the penis, so the erectile tissue fills more easily during arousal. The difference is how long each one stays active in your body.
Tadalafil clears slowly. The Cialis label reports better erectile function than placebo for up to 36 hours after an as-needed dose, which is why it earned the “weekend pill” nickname. Vardenafil is closer to sildenafil in its timing. The vardenafil label has men take it about 60 minutes before sex, no more than once a day. The American Urological Association’s ED guideline puts its duration at up to 10 hours. See our vardenafil page for more on dosing ranges and who it suits.
Food, timing and the dissolving tablet
A heavy, fatty meal changes vardenafil more than tadalafil. The vardenafil label reports that high-fat meals cut its peak blood level by 18% to 50%, and the AUA guideline lists a high-fat meal as reducing its effect. Tadalafil is not affected by food. If your evenings usually start with a big dinner, that difference matters.
Vardenafil also comes as an orally dissolving tablet. MedlinePlus explains that it goes on the tongue right after you open the blister, without water, and that it comes in only one strength and cannot be swapped one for one with the regular tablet. Some men find that more discreet; others never notice a difference.
Side effects and the QT caution
Most side effects of both drugs come from PDE5 being present in blood vessels and muscles elsewhere in the body. Tadalafil is more associated with back pain and muscle aches, which typically show up 12 to 24 hours after a dose. Vardenafil is more associated with a stuffy nose. Both can cause headache and flushing.
Vardenafil has one caution tadalafil does not. In a heart-rhythm study, it lengthened the QT interval on an ECG by about as much as the antibiotic moxifloxacin. Its label tells men with congenital QT prolongation, and men taking class IA antiarrhythmics such as quinidine or procainamide or class III drugs such as amiodarone or sotalol, to avoid it. If you take any heart rhythm medicine, bring the bottle or a list to your visit.
Shared rules for both:

- Alpha-blockers need a stable dose first, then the lowest starting dose of the ED pill.
- Some antifungals, HIV drugs and antibiotics raise the levels of both and change the safe dose.
An erection lasting more than 4 hours, chest pain or sudden vision or hearing loss means stopping and getting emergency care through 911 or an ER.
Convenience and cost
Tadalafil’s main convenience is freedom from planning. With the daily dose, you do not time anything; with the as-needed dose, one tablet can cover a whole evening and the next day. The trade-off is that side effects can linger longer too. Men who already take tadalafil for urinary symptoms may be able to cover both problems with one daily tablet.
Vardenafil’s appeal is a shorter, more predictable window for men who dislike feeling a drug in their system the next day.
