Vardenafil (Levitra): Uses, Onset and Side Effects

Short answer

Vardenafil (Levitra) is an FDA-approved PDE5 inhibitor for erectile dysfunction, taken about 60 minutes before sex. It comes as film-coated tablets from 2.5 to 20 mg and as a 10 mg tablet that dissolves on the tongue, which is not interchangeable with the 10 mg pill. Men with long QT syndrome or on certain heart rhythm drugs should avoid it, and nitrates rule it out.

What vardenafil is and its FDA status

Vardenafil is a PDE5 inhibitor for erectile dysfunction, first approved by the FDA in 2003 under the brand name Levitra. Generic versions are now widely listed, and the film-coated tablet label covers 2.5 mg, 5 mg, 10 mg and 20 mg strengths.

Like the other ED pills, it does not start an erection on its own. Arousal releases nitric oxide, which raises cGMP and relaxes the blood vessels of the penis; vardenafil blocks the PDE5 enzyme that breaks cGMP down, so the response is stronger and lasts longer. Its half-life is about 4 to 5 hours.

Film-coated tablets vs the dissolving tablet

Vardenafil comes in two forms, and they are not the same dose even when the number matches. MedlinePlus describes both a regular tablet and a rapidly dissolving tablet that melts in the mouth without water.

The ODT label is explicit that the dissolving tablet delivers more drug than the 10 mg film-coated pill, so switching between them should go through your provider. The ODT suits men who have trouble swallowing pills or want something discreet without water. The film-coated tablet allows finer dose steps.

Why the milligram doses are so low

Vardenafil’s tablets top out at 20 mg, while sildenafil runs from 25 to 100 mg. That is because vardenafil is active at smaller amounts, not because it is weaker. Comparing ED drugs by the number on the tablet is misleading; what matters is whether the dose your provider chooses works for you with tolerable side effects.

Low doses also explain why interactions bite hard. Ritonavir raised vardenafil exposure 49-fold in testing, so the label caps the dose at 2.5 mg in 72 hours for men on that drug, and other strong CYP3A4 inhibitors carry similar limits. Men 65 and older reach higher blood levels, which is why the label suggests starting them at 5 mg.

The QT caution and other safety rules

Vardenafil carries a heart rhythm caution that the other ED pills do not stress in the same way. In a study of 59 healthy men, both a normal 10 mg dose and an 80 mg dose lengthened the QT interval on the ECG by a similar amount to the antibiotic moxifloxacin, a drug known to do so. The clinical impact is unclear, but the label says men with congenital long QT syndrome, and men taking Class IA antiarrhythmics such as quinidine or procainamide or Class III drugs such as amiodarone or sotalol, should avoid vardenafil.

The other absolute rules match the rest of the class:

  • No nitrates, ever. Nitroglycerin, isosorbide and poppers are contraindicated, as are riociguat and similar drugs. See ED pills and nitrates.
  • Alpha-blockers and blood pressure drugs. Men stable on an alpha-blocker should start at the lowest dose. Our page on ED pills with blood pressure medication covers the combinations.
  • Heart disease. Men with unstable angina, very low or uncontrolled high blood pressure, or a recent heart attack or stroke were not studied. Our guide for men with heart disease explains how that history shapes ED care.

The most common side effects in the label’s trials were headache (15% vs 4% on placebo), flushing (11% vs 1%), a runny or stuffy nose (9% vs 3%) and indigestion (4% vs 1%). Sudden vision or hearing loss, chest pain, or an erection lasting more than four hours needs 911 or an emergency room right away.

When is vardenafil tried after sildenafil?

Men usually reach vardenafil when sildenafil worked but its side effects were a problem, including the blue-tinted vision some men get at higher doses. In vardenafil’s own trials, vision changes did not reach the 2% reporting threshold, while sildenafil’s label reports abnormal vision in 11% of men at 100 mg. Those are separate studies, not a head-to-head test, and on paper vardenafil is only modestly more selective against the retinal enzyme PDE6 than sildenafil (more than 15-fold vs about 10-fold).

So a switch is reasonable to try, but it may not settle vision effects for every man. Headache and flushing, the other common reasons men stop sildenafil, also occur with vardenafil, so the trade is mainly about how your body responds to each. Is a 10 mg vardenafil pill weaker than 50 mg of sildenafil?
Not necessarily. Vardenafil is active at smaller amounts, so its doses are lower in milligrams. The number on the tablet is not a useful way to compare strength across different ED drugs.
Does vardenafil work with food?
The label allows it with or without food. High-fat meals lowered its peak level by 18% to 50% in testing, so a heavy meal right before a dose can make it feel weaker.
Who should not use the dissolving tablet?
The label says not to start vardenafil with the dissolving tablet in men on alpha-blockers, and not to use it with moderate or severe liver impairment or on dialysis. It also contains aspartame, which matters for anyone with phenylketonuria.
