Facial volume loss after GLP-1 weight loss

Why the face changes on GLP-1 medicines

Your face changes on semaglutide or tirzepatide because these medicines produce a lot of weight loss, and the face loses fat along with the rest of the body. The drugs do not attack facial tissue. The effect comes from how much and how quickly fat disappears.

The scale of loss is large. In the STEP 1 trial, adults on semaglutide 2.4 mg lost an average of 14.9% of body weight over 68 weeks, versus 2.4% on placebo. In SURMOUNT-1, the highest tirzepatide dose produced an average loss of 20.9% over 72 weeks. A loss of that size changes the face in ways a slower diet often spreads out over years.

Facial fat sits in separate pockets over the cheekbones, temples, under the eyes and along the jaw. When those pockets shrink, the skin that was stretched over them has less to rest on. The result reads as older or more tired, even when the man is healthier than he has been in years. A 2025 systematic review of the plastic surgery literature found that GLP-1 medicines cause facial changes resembling advanced aging, and that online searches for the term “Ozempic face” rose alongside searches for face filler.

Signs of facial volume loss

Men usually notice it in photos first. Common signs include:

  • Flatter cheeks, with the cheekbone more visible than the cheek pad
  • Hollow temples that make the brow and eyes look more prominent
  • Deeper smile lines from nose to mouth
  • Darker hollows under the eyes
  • Softer jaw edges or new jowls as skin loosens
  • A gaunt or drawn look after a long workday

Why timing matters: wait for weight to stabilize

Filler holds its shape most predictably when the face it is placed in has stopped changing. If you are still losing weight, more facial fat will leave after treatment and the result will drift, so you may need more product later to keep up.

That is why facial restoration is planned once your weight has been steady, typically after you reach a maintenance phase. Losing weight at a measured pace also helps. How the face is assessed

Assessment is an in-person look at your face from the front, sides and below, at rest and while talking. The nurse injector notes where volume is missing, how thick and elastic your skin is, and how your bone structure supports the soft tissue.

You will also be asked about:

  • Your current weight trend and GLP-1 dose
  • Previous filler, threads or facial surgery
  • Blood thinners, aspirin, fish oil and supplements that raise bruising risk
  • Cold sores, autoimmune conditions and past reactions to injections
  • Upcoming events, so treatment and settling time can be scheduled around them

Some men are asked to wait. Treatment is postponed during an active skin infection or cold sore outbreak near the area, and a history of severe allergic reactions or a bleeding disorder changes the plan or rules filler out. If any of these apply, mention them on the phone call so the visit is not wasted.

VOLUMA XC carries FDA approval for cheek augmentation and, since 2023, for moderate to severe temple hollowing in adults over 21. Sculptra was first approved in 2004 to restore facial fat loss in people with HIV and in 2023 gained approval for fine lines and wrinkles in the cheek region. Warning signs after filler that need the ER

The FDA warns that the most serious filler risk is unintended injection into a blood vessel, which can cause skin death, vision problems including blindness, and stroke. Call 911 or go to the ER right away for any change in vision, severe pain, skin that turns white, gray or dusky blue near the injection site, or stroke signs such as facial droop or weakness on one side. Trouble breathing or swelling of the lips and tongue also needs 911.

Milder redness, swelling and bruising are expected for a few days. Call the clinic about those during business hours.

Will my face fill back in if I regain some weight?
