Jowls in men

Short answer

Jowls form when the jaw and midface lose bone and fat support and the skin loosens, so tissue settles below the jawline. RADIESSE placed along the jaw can rebuild a straight edge, and Sculptra can thicken the skin over months by stimulating collagen. Mild to moderate jowls respond to these nonsurgical options, while heavy, loose skin may need a surgical opinion.

What jowls are and why men notice them

Jowls are the soft bulges that sit just below the jawline on either side of the chin. They break the straight line from the chin to the angle of the jaw, which is why men tend to notice them in side-on photos, video calls and the bathroom mirror when they look down.

On a male face the jawline carries a lot of weight. A crisp, straight edge reads as strong and rested. Once jowls interrupt it, the lower face can look heavier and older than the rest of you, even when your skin and fitness are good.

Signs and stages of jowling

Jowls usually develop slowly over years, and how far they have progressed decides which options make sense.

Stage What you see Usual fit
Early A slight dip in front of the jowl, the line still mostly straight Filler along the jaw and prejowl area
Moderate A visible bulge below the jaw, a shadow in front of it, a softer chin line Filler for structure plus Sculptra for skin quality
Advanced Hanging tissue, loose neck skin, folds that persist when you lift your chin Surgical opinion, with filler as a supporting option

Other signs that often travel with jowls include marionette lines running down from the corners of the mouth, flattening cheeks and a blurred jaw angle. When the angle and chin are the main problem rather than sagging tissue, our page on loss of jawline definition is the closer match.

What causes jowls: bone, fat and skin

Jowls come from three layers changing at once.

Bone. The jaw itself gives ground with age. A review of facial skeleton changes identifies the prejowl area of the lower jaw, along with the midface, as areas with a strong tendency to resorb over time. As bone recedes in front of the jowl, a dip forms and the jowl looks larger by contrast.

Fat. Facial fat sits in separate compartments rather than one sheet. In their anatomy study of facial fat compartments, Rohrich and Pessa found that jowl fat is the lowest of these compartments. As the cheek compartments above it deflate and shift, the jowl compartment becomes more prominent.

Skin and ligaments. Collagen and elastin decline, the skin thins, and the supports holding tissue in place loosen. Sun exposure, smoking, genetics and large weight changes all speed that process. Men who lose a lot of weight quickly on GLP-1 medicines often notice jowls appear, a pattern covered on our page about facial volume loss after GLP-1 weight loss.

How a jowl assessment works

A jowl assessment is an in-person look at your face from several angles, at rest and while you talk and smile. The nurse injector checks bone support at the chin and jaw angle, how much fat sits in the jowl itself, how thick and elastic your skin is, and whether the cheeks above have lost volume.

You will also be asked about blood thinners and supplements that increase bruising, prior filler or surgery, cold sores, autoimmune conditions and past reactions to injections. Photos are taken for your record so later visits can be compared fairly.

Expect a conversation about trade-offs as well. Filler that straightens the jaw edge adds a little width to the lower face, which suits most men but not every face shape, and the injector will show you in the mirror where product would go before anything is placed.

The honest outcome of some assessments is a referral. When skin is very loose, filler alone can add weight without lifting, and a surgical consultation is the better first step.

Treatment options: RADIESSE, Sculptra and support from above

Nonsurgical treatment rebuilds what has been lost around the jowl so the line straightens, rather than pulling the jowl up.

  • RADIESSE along the jaw. RADIESSE is calcium hydroxylapatite, a firm filler suited to deep placement on bone. The FDA approved RADIESSE (+) in 2021 for deep injection to improve moderate to severe loss of jawline contour in adults over 21. Placed in the prejowl dip and along the jaw edge, it camouflages the jowl by restoring the straight line around it. See RADIESSE for men.
  • Sculptra for collagen. Sculptra is poly-L-lactic acid that stimulates your own collagen over a series of sessions. It suits men whose skin has thinned and lost firmness across the lower face. Change is gradual, which many men prefer.
  • Support from above. When the cheeks have deflated, restoring a little cheek volume can reduce the heaviness that settles toward the jowl.

The American Academy of Dermatology lists typical durability as 6 months to 1 year for calcium hydroxylapatite and 1 to 3 years for poly-L-lactic acid, though it varies by person and area. Our comparison of RADIESSE and Sculptra and our answer on how long jawline filler lasts go further.

When to seek emergency care after filler

According to the FDA, the most serious filler risk is injection into a blood vessel, which can damage skin, affect vision and cause stroke. Call 911 or go to the ER for sudden vision change, severe or worsening pain, skin that turns pale, mottled or dusky near the treatment area, facial droop or one-sided weakness, or swelling of the lips and tongue with trouble breathing.

Expected effects such as mild swelling, tenderness and bruising settle over days. For those, a phone call to the clinic is the right step, and any lump that stays firm after two weeks deserves a follow-up visit.

How Ultimate Male treats jowls

At Ultimate Male, jowl treatment starts with a free 10-minute phone call, then a consultation at our San Gabriel or Downey clinic with aesthetic nurse Sabrina Sher or Paul Pham. The goal is a masculine result: a straighter, firmer jaw edge, not a rounder lower face.

Your plan names each area, the product for it and whether Sculptra sessions are included. The amount of product depends on your bone structure and how advanced the jowls are, so it is set at the consultation instead of quoted ahead of time. You pay for dermal fillers visit by visit, and CareCredit, Prosper Healthcare Lending and Afterpay can spread the cost. Fillers for appearance are generally not FSA or HSA eligible, and you receive an itemized receipt either way.

Jowls in men

Still unsure? Take the free assessment

Can a beard hide jowls?
A full beard can soften the line, and many men use one that way. It does not change the underlying structure, and grey or patchy growth can make the jaw look less defined, so some men combine grooming with treatment.
Can RADIESSE be dissolved if I do not like it?
Not in the way hyaluronic acid filler can. RADIESSE is calcium hydroxylapatite, which hyaluronidase does not break down, so conservative placement and careful planning matter more.
Do jowls come back after filler?
The aging and volume loss behind jowls continue, and the body gradually breaks down filler. Most men plan maintenance visits, with timing that depends on the product and how they respond.
