Why under-eye hollows make you look tired
Under-eye hollows make you look tired because they create a shadow, and the brain reads that shadow as fatigue. The hollow sits along the tear trough, the groove that runs from the inner corner of the eye down and out toward the cheek.
For men the effect can be frustrating. You sleep well, train, eat well, and colleagues still ask if you are worn out. The problem is structural, so more sleep does not fix it.
Hollow, puffy or loose: telling them apart
Three different problems all get called bags or dark circles, and only one of them responds well to filler.

A quick mirror test helps. Look straight ahead under a ceiling light. If you see a shadow in a groove that softens when you tilt your face up toward the light, a hollow is likely. If a soft pouch sits above the groove and looks worse after a salty dinner or a late night, puffiness is a bigger part of the picture.
What causes hollows
Hollows come from changes in bone, fat and skin around the eye.
The bony rim of the eye socket widens with age. A review of aging in the facial skeleton identifies the lower outer part of the orbital rim as an area that resorbs over time, which leaves less support for the tissue below the eye. Anatomy work on facial fat compartments shows that fat around the eye sits in separate compartments bounded by thin partitions, so the area can hollow in one spot and bulge in another.
Other contributors include:
- Genetics, since some men have deep-set eyes or a hollow from their twenties
- Significant weight loss, including rapid loss on GLP-1 medicines
- Thin skin that lets underlying shadows and veins show
- Sun damage, smoking and allergies that leave the area irritated

When filler helps and when it will not
Filler helps when there is a true hollow, the skin over it is reasonably firm, and there is little puffiness above it. A small amount of soft hyaluronic acid filler placed carefully along the trough can lift the shadow without adding bulk.
It is a poor choice in a few situations:
- Prominent bags. Adding volume under a bulging fat pad can make the whole area look fuller and puffier.
- Fluid swelling. Lids that swell easily can hold onto fluid around filler, which shows as a lasting puffy ridge.
- Very loose or crepey skin. Filler does not tighten skin, and thin skin can show product as a bluish tint.
- Cheek-driven shadows. When the cheek below has deflated, supporting the cheek first may soften the under-eye without treating it directly, the same logic used for deep smile lines.
Regulation is worth knowing. The FDA’s general dermal filler page lists the area around the eyes among areas where fillers in general are not approved, while specific hyaluronic acid products have since gained approval for this use. One example is JUVÉDERM VOLBELLA XC, approved in 2021 for improving infraorbital hollowing in adults over 21. Which product suits your under-eye is decided at the consultation.
The American Academy of Dermatology puts typical durability for hyaluronic acid gels at 4 to 12 months. Under-eye filler is also reversible with hyaluronidase, which is a safety advantage in a delicate area.
How the under-eye is assessed
Assessment is an in-person look at the eye area in different lighting, at rest and while smiling, from the front and both sides. The nurse injector checks how deep the groove is, how much the fat pad bulges, how thin the skin is, and how the cheek below supports the lid.
You will be asked about blood thinners and supplements that raise bruising risk, prior eye surgery or filler, cold sores, allergies and any thyroid eye problems. If the main issue is skin texture or pigment rather than volume, the plan may point to the VI Peel for men instead, and fine lines at the outer corner respond to BOTOX for crow’s feet.
Some under-eye changes are medical, not cosmetic. Swelling that appears suddenly, affects one eye, or comes with bulging eyes, double vision or redness needs a medical evaluation before anything cosmetic is considered.
Vision changes and other emergency signs
The area around the eye carries the highest stakes for filler. The FDA warns that filler entering a blood vessel can cause vision problems including blindness, skin damage and stroke. Call 911 or go to the ER right away for any blurred vision, vision loss or eye pain after treatment, skin that turns pale or dusky, severe headache, facial droop or trouble breathing.
