Signs estrogen may be running high
Estrogen is not only a women’s hormone. Men need estradiol, the main form, for bone strength, libido and erections. Trouble starts when there is too much of it relative to testosterone. Common signs include:
- Breast tenderness, puffiness behind the nipple, or true breast gland growth.
- Lower sex drive and weaker erections.
- Fertility problems.
- Fatigue and low mood, often alongside a low testosterone.

Where a man’s estrogen comes from
Men make small amounts of estradiol directly in the testes. Most of the rest comes from converting testosterone through an enzyme called aromatase. MedlinePlus Genetics notes that in males, aromatase is most active in fat tissue.
That single fact explains a lot. More body fat means more aromatase, which means more testosterone turned into estradiol. The extra estradiol then signals the brain to slow testosterone production, so a man can end up with high estradiol and low testosterone at the same time. Three common drivers when you are not on TRT
Excess body fat
For most men who are not on TRT, the main driver is weight around the middle. It is also the most fixable. As fat comes off, aromatase activity falls, estradiol tends to drop and testosterone often recovers. Regular alcohol
Alcohol affects both sides of the balance. A 2024 meta-analysis of 21 studies with 10,199 men found that chronic alcohol consumption was linked to lower total and free testosterone and higher estradiol. Liver strain
The liver clears hormones from the blood, so when it is struggling, estrogen effects build. MedlinePlus lists chronic liver disease among the causes of male breast enlargement. Fatty liver from weight, metabolic problems or alcohol is common in men and often silent, which our fatty liver disease page explains.
Less common causes include certain medicines, an overactive thyroid and, rarely, tumors. MedlinePlus lists estrogen-producing tumors and testicular or adrenal cancers among them.
Why the type of estradiol test matters
This is the part most men never hear about. Men’s estradiol runs far lower than a premenopausal woman’s, and many standard lab tests were designed for the higher range.
The Endocrine Society’s position statement on estradiol measurement found that current direct immunoassays cannot measure estradiol below 30 pg/mL with reasonable precision, and that men’s levels sit at or below the sensitivity of many direct assays. It notes that mass spectrometry methods have greater specificity and sensitivity, which is especially important at the low concentrations seen in men. In one study it reviewed, an immunoassay reported healthy men’s estradiol at about 33 pg/mL while other methods measured about 21 pg/mL.

In practice, a standard test can make a normal man look high. Before acting on an elevated estradiol, it is worth confirming it with a sensitive method. What else gets checked
A high estradiol is only meaningful next to the rest of the picture. A useful workup adds total and free testosterone and SHBG to see the balance, LH and FSH to see how the brain is responding, prolactin and thyroid tests, and a liver panel. MedlinePlus explains that liver function tests measure enzymes and proteins such as ALT, AST, ALP, albumin and bilirubin to show how well the liver is working. Why not simply block estrogen?
It is tempting to treat a high number with a pill that lowers it. Aromatase inhibitors such as anastrozole do lower estradiol, and they have a place in some men on testosterone therapy, which our anastrozole on TRT page discusses. For a man whose estradiol is high because of weight, alcohol or liver strain, blocking estrogen leaves the cause in place.
There is also a real risk of overshooting. Estradiol that falls too low can weaken bones, flatten libido and make erections worse, the same complaints that brought many men in. That is why the cause comes first, and why any estrogen-lowering medicine is reserved for clear reasons with follow-up labs.
When to get seen quickly
Most high estrogen is a slow problem. Some signs need prompt care: a hard, fixed breast lump on one side, bloody nipple discharge, or a new lump in a testicle should be checked within days. Yellowing skin or eyes, very dark urine, or swelling of the belly can mean serious liver trouble and need same-day care. Vomiting blood or confusion in someone with liver disease is an emergency; call 911.
