As of October 8, 2026, NovoCare lists Wegovy pens at $349 a month for standard doses and $399 for the 7.2 mg HD pen, with $199 for the first two starting fills for new patients. Wegovy pills step up with the dose, from $149 to $299 a month. A clinic program adds the evaluation, labs, scans and check-ins on top of the medicine.

How much does semaglutide cost without insurance?

The brand medicine has a published self-pay price, and it is the clearest starting point. As of October 8, 2026, NovoCare, Novo Nordisk’s pharmacy program, lists these monthly prices for Wegovy:

  • Pens: $349 a month for the 0.25, 0.5, 1, 1.7 and 2.4 mg doses, and $399 for Wegovy HD 7.2 mg. New patients pay $199 for each of their first two monthly fills at the 0.25 and 0.5 mg doses, an offer that runs through December 31, 2026.
  • Pills: $149 a month for 1.5 mg, $199 for 4 mg, and $299 for 9 mg and 25 mg.

These prices cover the medicine only. Clinical care, labs and follow-up are separate, and Novo Nordisk says it can change the program at any time.

How the price changes with each dose step

Semaglutide starts low and steps up, and the price follows a different pattern for the pen and the pill. The Wegovy label moves the pen up every four weeks, while the tablet steps up every 30 days.

For a new patient on the pen, the bill rises once, in month three, then holds flat unless you move to the HD dose. On the pill, it rises with each early step and levels off at $299. The label also allows your provider to delay a step by four weeks if a dose is hard to tolerate, which stretches out the lower-priced months. What a clinic program adds to the medicine

The medicine is one part of the cost. A supervised program adds a medical evaluation, lab work, a body composition analysis and regular check-ins, which is where dose decisions, side effects and muscle loss get managed.

What about compounded semaglutide?

Compounded versions are no longer a routine lower-cost option. The FDA declared the semaglutide shortage resolved in February 2025, and its policy update for compounders explains that copies of an approved product are now limited to individual patients with a documented clinical difference.

That narrow route still exists, but it starts with a medical reason, not a price. Using FSA or HSA funds

Semaglutide for weight loss can be paid with pre-tax money when it treats a diagnosed condition. IRS Publication 502 allows medicines that need a clinician’s order, and it allows weight loss treatment when a physician has diagnosed a disease such as obesity or hypertension.

Do self-pay prices change during the year?
They can. Novo Nordisk states it may change or end its self-pay program at any time, and the new-patient offer has a stated end date, so check the price again before each refill.
Does a higher dose always mean better results?
Not necessarily. The label lets your provider choose a maintenance dose based on response and tolerability, and some men do well on a lower step.
